    Side Effects & Safety

    In the 21-day phase 1 study in healthy young men, adverse event frequency was similar to placebo and no drug-related serious adverse events occurred, but total testosterone, sex hormone binding globulin, HDL cholesterol and triglycerides fell dose-dependently (PMID: 22459616). Outside research settings the picture is worse: biopsy-confirmed cholestatic liver injury has been reported repeatedly (PMID: 32637435, PMID: 36257328, PMID: 39421081), including cases with ductopenia and recovery over about three months (PMID: 34141767). A hospital case series of liver injury from anabolic steroids, SARMs and bodybuilding supplements reported a median 175 days to normal liver biochemistry and one liver transplant among 23 patients (PMID: 38372012). A single-subject report of ligandrol with MK-677 recorded large rises in liver enzymes and falls in HDL cholesterol and testosterone (PMID: 36303408). In male rats, endurance fell and lipid and hormonal profiles worsened (PMID: 40087185).

    Contraindications

    Mechanism-based: contraindicated in pregnancy because androgen receptor agonists can virilize a female fetus, and in men with known or suspected prostate cancer. Existing liver disease or raised transaminases is a case-report-based reason to avoid it (PMID: 32637435, PMID: 36257328). Because it suppresses endogenous testosterone and gonadotropins (PMID: 22459616), it works against men being treated for infertility. Prohibited for athletes in tested sport at all times (PMID: 28137616).
