Canagliflozin is a sodium-glucose cotransporter 2 (SGLT2) inhibitor, a C-glucoside discovered by chemists at Mitsubishi Tanabe Pharma, that lowers blood glucose in type 2 diabetes by making the kidneys excrete glucose in the urine; the FDA approved canagliflozin in 2013 as Invokana.

01Overview
What Canagliflozin is
In people with type 2 diabetes, canagliflozin lowered the rate of major cardiovascular events in the CANVAS Program and of kidney failure outcomes in the CREDENCE trial, but CANVAS also found about twice the rate of lower limb amputations, and the US label carried a boxed amputation warning from July 2017 to August 2020 (PMID: 28605608, PMID: 30990260; FDA approval letters for Invokana supplements 26 and 34). In the US National Institute on Aging's Interventions Testing Program, canagliflozin in the diet from 7 months of age extended median lifespan by 14 percent in male mice but not in female mice, and a start at 16 months lengthened male lifespan by 14 percent while shortening female lifespan by 6 percent (PMID: 32990681, PMID: 38753230). No human trial has measured an effect of canagliflozin on aging or lifespan.
How it works
How canagliflozin lowers blood glucose
SGLT2 is a transporter in the proximal tubule of the kidney that reabsorbs most of the glucose filtered from the blood; canagliflozin inhibits SGLT2, lowers the renal threshold for glucose (the blood level above which glucose spills into urine) and increases urinary glucose excretion (US prescribing information for Invokana). In phase 1 studies described in the label, canagliflozin lowered the renal threshold in people with type 2 diabetes from about 240 mg/dL to about 70 to 90 mg/dL at the 300 mg dose and raised mean urinary glucose excretion to about 100 g a day at 100 or 300 mg (US prescribing information for Invokana). Blocking SGLT2 also sends more sodium to the distal tubule, which the label states is believed to increase tubuloglomerular feedback and reduce pressure inside the glomeruli, the kidney's filtering units (US prescribing information for Invokana).

Canagliflozin, dapagliflozin and empagliflozin were derived from the natural product phlorizin (PMID: 27381369). Mitsubishi Tanabe chemists reported in 2010 that C-glucosides carrying a heteroaromatic ring were more metabolically stable SGLT2 inhibitors than the earlier O-glucoside T-1095, and described the thiophene derivative canagliflozin as a highly potent and selective SGLT2 inhibitor (PMID: 20690635).
A second target in the gut (human data)
Canagliflozin is also a low-potency inhibitor of SGLT1, the main glucose transporter of the small intestine (PMID: 23412078). In a placebo-controlled crossover study of 20 healthy adults led by Janssen researchers, canagliflozin 300 mg taken before a mixed meal delayed the appearance of meal glucose in the blood and reduced the 2-hour rise in glucose by 35 percent and in insulin by 43 percent (PMID: 23412078).
Mitochondria and AMPK (cell and mouse data)
In cultured cells, canagliflozin, but not dapagliflozin, empagliflozin or phlorizin to any significant extent, activated AMP-activated protein kinase (AMPK) by inhibiting complex I of the mitochondrial respiratory chain, at concentrations measured in human plasma in clinical trials (PMID: 27381369). Oral canagliflozin activated AMPK in mouse liver but not in muscle, fat or spleen (PMID: 27381369).
Proposed reasons for the mouse lifespan effect (animal data)
The Interventions Testing Program authors proposed that canagliflozin extends male mouse lifespan by blunting peak glucose levels, and they noted that acarbose, which blocks glucose surges by a different route, has a similar male-biased effect (PMID: 32990681). Canagliflozin also lowered fasting glucose and improved glucose tolerance in female mice, which did not live longer (PMID: 32990681). In a later cohort, blood canagliflozin levels were about 20 times higher in aged female mice than in young male mice, which the authors proposed as one possible explanation for harm to females when treatment started late (PMID: 38753230).
In 30-month-old mice, canagliflozin improved insulin sensitivity in the hypothalamus and hippocampus of males, reduced age-related hypothalamic gliosis in both sexes, and reduced hippocampal microglial and astrocyte activation in males but not in females (PMID: 35707855). Much of this brain work comes from Marianna Sadagurski's group at Wayne State University (PMID: 35707855, PMID: 41047765). These are mouse findings; whether any of them applies to people is unknown.

Who should not use it
Contraindications
The label lists a serious hypersensitivity reaction to canagliflozin as a contraindication (US prescribing information for Invokana).
Read all the contraindications and cautionsShow less
In the CANVAS trials, amputation risk was highest in people with a previous amputation, peripheral vascular disease or neuropathy, and lower limb infections, gangrene and diabetic foot ulcers were the most common events leading to amputation (US prescribing information for Invokana; PMID: 30868176).
Canagliflozin is not indicated for glycemic control in type 1 diabetes, where it markedly raises the risk of diabetic ketoacidosis, and the label lists reduced calorie intake, a ketogenic diet, acute febrile illness, surgery, volume depletion, alcohol abuse and insulin dose reduction as conditions that can precipitate ketoacidosis (US prescribing information for Invokana).
The label does not recommend canagliflozin for glycemic control when eGFR is below 30 mL/min/1.73 m2, and lists impaired kidney function, older age and loop diuretics as factors that raise the risk of volume depletion or low blood pressure (US prescribing information for Invokana).
Based on kidney findings in juvenile rats, the label does not recommend canagliflozin during the second and third trimesters of pregnancy or while breastfeeding (US prescribing information for Invokana).
Canagliflozin has not been studied in severe hepatic impairment, and the label does not recommend it in that group (US prescribing information for Invokana).
The label advises considering factors that contribute to fracture risk, based on the CANVAS fracture finding (US prescribing information for Invokana).
For aging use, the lifespan results come from mice, and a late start shortened female mouse lifespan (PMID: 38753230); no human trial has measured an effect of canagliflozin on aging.
Interactions
5 pairings in our record: 3 with a caution and 2 to monitor.
UGT enzyme inducers (rifampin, phenytoin, phenobarbital, ritonavir)
CautionInducers of UGT enzymes, such as rifampin, phenytoin, phenobarbital and ritonavir, lower canagliflozin exposure, which the label states may reduce its effectiveness (US prescribing information for Invokana).
Insulin or sulfonylureas
CautionThe label states that canagliflozin used with insulin or an insulin secretagogue such as a sulfonylurea increases the risk of hypoglycemia (US prescribing information for Invokana).
Loop diuretics
CautionThe label lists loop diuretics, with impaired kidney function and older age, as factors that raise the risk of volume depletion or low blood pressure with canagliflozin (US prescribing information for Invokana).
Digoxin
MonitorCanagliflozin increases digoxin exposure, and the label calls for monitoring people who take both (US prescribing information for Invokana).
Lithium
MonitorThe label states that SGLT2 inhibitors such as canagliflozin may lower serum lithium concentrations (US prescribing information for Invokana).

08Questions
Frequently asked questions
What is canagliflozin?
Canagliflozin is a sodium-glucose cotransporter 2 (SGLT2) inhibitor that lowers blood glucose by blocking glucose reabsorption in the kidney, so more glucose leaves the body in urine (US prescribing information for Invokana). Canagliflozin was reported in 2010 by Mitsubishi Tanabe Pharma chemists as a highly potent and selective SGLT2 inhibitor (PMID: 20690635). The FDA approved canagliflozin as Invokana on 29 March 2013 (Drugs@FDA NDA 204042).
Does canagliflozin extend lifespan?
Canagliflozin in the diet from 7 months of age extended the median lifespan of male mice by 14 percent in the NIA Interventions Testing Program, with no lifespan benefit in female mice (PMID: 32990681). Canagliflozin started at 16 months of age extended male mouse lifespan by 14 percent but shortened female mouse lifespan by 6 percent (PMID: 38753230). No human trial has tested whether canagliflozin affects lifespan or aging.
Why did canagliflozin carry a boxed warning about amputations?
Canagliflozin was linked to about twice the rate of lower limb amputations seen with placebo in the CANVAS Program (hazard ratio 1.97), mostly of the toe or metatarsal (PMID: 28605608). The US canagliflozin label carried a boxed warning for lower limb amputation from 25 July 2017 until the FDA approved its removal on 18 August 2020 (FDA approval letters for Invokana supplements 26 and 34). The CREDENCE trial found no significant difference in amputations with canagliflozin, and a pooled analysis suggested that the CANVAS finding may have been due to chance (PMID: 30990260, PMID: 32436638). Lower limb amputation is still listed as a warning on the current canagliflozin label (US prescribing information for Invokana).
What did the CANVAS and CREDENCE trials find?
In the CANVAS Program of 10,142 adults with type 2 diabetes and high cardiovascular risk, canagliflozin lowered the rate of cardiovascular death, heart attack or stroke (hazard ratio 0.86) (PMID: 28605608). In the CREDENCE trial of 4,401 adults with type 2 diabetes and albuminuric kidney disease, canagliflozin lowered the risk of kidney failure, creatinine doubling or renal or cardiovascular death by 30 percent, and the trial was stopped early after a planned interim analysis (PMID: 30990260). Janssen funded both canagliflozin programs (PMID: 28605608, PMID: 30990260).
What are the main side effects of canagliflozin?
In pooled 26-week trials, canagliflozin raised the rate of genital fungal infections to about 11 percent in women and about 4 percent in men, against 2.8 and 0.7 percent on placebo (US prescribing information for Invokana). Canagliflozin also carries label warnings for ketoacidosis, volume depletion and acute kidney injury, genitourinary infections including Fournier's gangrene, lower limb amputation and bone fracture (US prescribing information for Invokana).
Why did canagliflozin help only male mice?
The reason canagliflozin extended lifespan only in male mice is unknown, and canagliflozin lowered fasting glucose and improved glucose tolerance in female mice as well (PMID: 32990681). Blood canagliflozin levels were about 20 times higher in aged female mice than in young male mice, which the investigators proposed as one possible reason for the late-start harm in females (PMID: 38753230). End-of-life pathology in canagliflozin-treated mice showed no clear pattern that explained the sex difference (PMID: 40601216).
What is the regulatory status of canagliflozin?
Canagliflozin is an FDA-approved prescription drug, approved as Invokana on 29 March 2013 for glycemic control in type 2 diabetes and later for cardiovascular and kidney risk reduction in adults with type 2 diabetes (Drugs@FDA NDA 204042; FDA approval letters). Canagliflozin is also approved in tablets combined with metformin, and in December 2024 the FDA extended the canagliflozin glycemic indication to children aged 10 years and older (Drugs@FDA; FDA approval letter, NDA 204042 supplement 43). In the EU, canagliflozin has held a marketing authorisation as Invokana since 15 November 2013 (EMA, Invokana product page). The canagliflozin label has no indication for aging or lifespan (US prescribing information for Invokana).
