Minoxidil is an FDA-approved treatment for pattern hair loss. Originally an oral blood-pressure drug, its hair-growth side effect led to the topical products (Rogaine) at 2% and 5%. It is a potassium-channel-opening vasodilator and a prodrug -- activated in the scalp by sulfotransferase to minoxidil sulfate -- that prolongs the hair growth (anagen) phase. Topical minoxidil is available over the counter; low-dose oral minoxidil is used off-label for hair loss under prescription.
What to Expect
- •Early shedding in weeks 2-8: minoxidil pushes resting follicles into a new cycle, so older hairs fall out before new ones grow in. It is temporary and usually a sign the drug is working -- do not stop.
- •Regrowth is slow. Give it at least 4 months of consistent daily use before judging, with peak benefit around 12 months.
- •It is maintenance, not a cure. Stop and gains are typically lost within 3-6 months as hair returns to its untreated trajectory.
- •Best response is on the crown (vertex); frontal and temple recession responds less.
- •Roughly a third of users respond poorly, likely due to low follicular sulfotransferase activity [PMID:34159872].

Is minoxidil FDA-approved?
Topical minoxidil (2% and 5%) is FDA-approved and sold over the counter for pattern hair loss in men and women (the 5% foam was approved for women in 2014). Oral minoxidil is FDA-approved only as a blood-pressure drug (Loniten); using low-dose oral minoxidil for hair loss is off-label and requires a prescription.
Topical or oral minoxidil -- which is better?
For most people topical is the first choice because it is non-prescription and avoids systemic side effects. In a head-to-head randomized trial, oral 5 mg/day was not superior to topical 5% twice daily in men over 24 weeks [PMID:38598226]. Low-dose oral is mainly for people who cannot tolerate the topical or will not apply it consistently.
Why is my hair falling out MORE after starting minoxidil?
This early shed is expected. Minoxidil pushes resting (telogen) follicles into a new growth phase, so older hairs are ejected before new ones grow in. It usually happens in weeks 2-8 and then settles down -- generally a sign the drug is working, so do not stop.
How long until I see results, and do I have to use it forever?
Give it at least 4 months of daily use, with peak results around 12 months. Minoxidil is maintenance, not a cure -- if you stop, gains are typically lost within 3-6 months and hair returns to its untreated trajectory.
Why does minoxidil not work for some people?
Minoxidil is a prodrug that has to be activated by a scalp enzyme (sulfotransferase, SULT1A1). People with low follicular sulfotransferase activity convert less of it and tend to respond poorly, which is thought to explain a large share of non-responders [PMID:34159872].
Is low-dose oral minoxidil safe?
In a multicenter study of 1404 patients, low-dose oral minoxidil was well tolerated: the most common effect was unwanted hair growth (~15%), while systemic effects like lightheadedness, fluid retention, and fast heart rate each occurred in under 2%, and only 1.7% stopped because of side effects [PMID:33639244]. It is still a prescription vasodilator, so it should be taken under medical supervision with blood-pressure monitoring.
Can women use minoxidil?
Yes. The 5% foam once daily and the 2% solution twice daily are FDA-approved for female pattern hair loss, and randomized trials show 5% is effective, with once-daily 5% foam non-inferior to twice-daily 2% solution and better tolerated [PMID:15034503][PMID:21700360]. Women should avoid the twice-daily 5% solution because of higher facial-hair risk.
