01Overview
What ISRIB is
ISRIB works by binding and activating eIF2B, the enzyme that recycles the translation initiation factor eIF2, which cryo-electron microscopy later placed in a cleft at the symmetry interface of the human eIF2B decamer (PMID: 25875391, PMID: 29599213, PMID: 29599245). Mice given ISRIB after training performed better on hippocampal memory tasks, and later work reported reversal of memory deficits after brain injury and in old mice (PMID: 23741617, PMID: 28696288, PMID: 33258451). No human study of ISRIB is registered on ClinicalTrials.gov, and chemists working on the same target wrote that the poor solubility of ISRIB limits its absorption and its advancement, which is why newer eIF2B activators rather than ISRIB itself went into clinical trials (PMID: 42153306).
How it works
The pathway ISRIB acts on
Four kinases (PERK, PKR, GCN2 and HRI) respond to different stresses by adding a phosphate to serine 51 of the alpha subunit of eIF2. That single change turns eIF2 from a substrate of its guanine nucleotide exchange factor eIF2B into a competitive inhibitor of it, which slows protein synthesis across the cell while a short list of messenger RNAs, including ATF4, is translated more. The four pathways together are called the integrated stress response (PMID: 23741617, PMID: 32327570).

How ISRIB was found
The Walter laboratory built a reporter in which firefly luciferase sits behind the 5' untranslated region of ATF4, treated cells with thapsigargin to switch on PERK, and screened 106,281 compounds. That gave 460 hits, 187 of which were specific to the PERK branch rather than the IRE1 branch, 77 that survived a second reporter in a different cell line, and 28 that blocked endogenous ATF4 on a western blot. One of those 28 became ISRIB (PMID: 23741617).
The isomer matters
ISRIB is a symmetric bis-glycolamide built around a di-substituted cyclohexane, so it exists as two diastereomers. The trans isomer inhibited the ATF4 reporter with a half-maximal concentration of 5 nanomolar against 600 nanomolar for the cis isomer, a hundredfold difference, and all of the work in the original paper used the synthesised trans isomer (PMID: 23741617). Papers that write simply "ISRIB" usually mean trans-ISRIB.
The target: eIF2B
ISRIB does not block the kinases. A short hairpin RNA screen and biochemistry identified eIF2B as the target and showed that ISRIB stabilises activated eIF2B dimers, with the delta subunit contributing to the binding site (PMID: 25875391). Independently, a group in Cambridge selected cells resistant to ISRIB and found clustered missense mutations in the N-terminal part of the eIF2B delta subunit, which reversed both the cellular effect and the stimulation of eIF2B exchange activity when reintroduced by gene editing (PMID: 25858979). Two 2018 cryo-electron microscopy structures then showed where it sits: one reported an atomic-resolution structure with ISRIB in a deep cleft of the decamer, cross-bridging a central symmetry interface and promoting assembly of two tetramers into the fully active enzyme (PMID: 29599213), and the other resolved the molecule at the interface between the beta and delta regulatory subunits at 4.1 angstroms (PMID: 29599245).
ISRIB only works inside a window
Two findings limit what ISRIB can do. Work in picornavirus-infected and chemically stressed cells showed that ISRIB rescues translation when phosphorylated eIF2 is low, and loses that ability once phosphorylated eIF2 passes a threshold, so the drug cannot switch off an acute, full-strength stress response (PMID: 30674674, with a 2021 correction to its supporting information at PMID: 33875607). Later biochemistry found that ISRIB accelerates eIF2B activity mainly when phosphorylated eIF2 is present, and that phosphorylated eIF2 and ISRIB oppose each other's binding to eIF2B, which the authors described as antagonistic allostery (PMID: 33220178). In prion-infected mice ISRIB restored protein synthesis only to about 70 percent of normal, against about 100 percent for a PERK kinase inhibitor, and the authors treated that partial effect as the reason ISRIB spared the pancreas (PMID: 25741597).
What the memory claim rests on
In the original paper, mice injected after each water maze trial reached the hidden platform faster and preferred the target quadrant in the probe test, mice given a single injection after fear conditioning froze more at 24 hours but not at 1 hour, and rats given ISRIB directly into the amygdala froze more to a tone at 24 hours but not at 3 hours (PMID: 23741617). The selective effect on long-term and not short-term memory is the pattern the authors used to argue that ISRIB acts on memory consolidation.

ISRIB side effects and interactions
Side effects
Common
- No human adverse event data exist for ISRIB
- The original paper reported no overt toxicity in mice at the doses used (PMID: 23741617)
- Wild-type C57BL/6J mice tolerated 5 mg/kg once daily for 62 consecutive days (PMID: 28721361)
- Daily dosing in prion-infected mice left pancreatic weight and structure normal and blood glucose unchanged (PMID: 25741597)
Serious events
- Daily dosing at 5 mg/kg was stopped after eight days in an APP amyloid cohort because of a significant rise in mortality; seven mice died in the second week, all in ISRIB groups, and four of them were non-transgenic (PMID: 28721361)
- Prion-infected mice treated daily reached 20 percent body weight loss by about 96 days, which the authors could not attribute to pancreatic damage or diabetes (PMID: 25741597)
- ISRIB reduced the viability of cells under sustained endoplasmic reticulum stress, and is deliberately cytotoxic in some tumour models (PMID: 23741617, PMID: 29720449)
- No human pharmacokinetic, long-term, reproductive or interaction data exist
Who should not use it
Contraindications
No human data at all. ISRIB has not been given to people in any registered study, so there is no basis for describing how a person would respond to it (ClinicalTrials.gov search, 25 October 2026).

Conditions where the stress response is protective. ISRIB reduced the viability of cells under sustained endoplasmic reticulum stress (PMID: 23741617), and blocking the response is deliberately cytotoxic in some tumour models (PMID: 29720449, PMID: 35114585). A compound that removes a protective brake on protein synthesis is not neutral in a cell that needs that brake.
Mice of an APP amyloid line died on daily dosing. Daily ISRIB at 5 mg/kg had to be stopped after eight days in an APP amyloid cohort because of a significant rise in mortality, and four of the seven deaths were in non-transgenic mice of that line. The same dose did not do this in C57BL/6J or PS19 mice (PMID: 28721361).
Acute, strong stress states. ISRIB cannot suppress the integrated stress response once phosphorylated eIF2 passes a threshold, so its effects depend on how strongly the pathway is switched on (PMID: 30674674, PMID: 33220178).
Not a medicine. ISRIB is a research chemical with no approval anywhere and no clinical indication.
Interactions
4 pairings in our record: 3 with a caution and 1 to monitor.
-
PERK kinase inhibitors
CautionISRIB and a PERK kinase inhibitor act at different points of the same pathway. In prion-infected mice both were neuroprotective, but the PERK inhibitor GSK2606414 restored protein synthesis fully and destroyed exocrine pancreatic tissue, while ISRIB restored it partially and left the pancreas intact (PMID: 25741597).
-
Proteasome inhibitors
CautionIn breast cancer cells, ISRIB added to the proteasome inhibitor bortezomib pushed cells into paraptosis by restoring translation, while in myeloma cells it protected against bortezomib-induced apoptosis (PMID: 35114585). The direction of the effect depended on the cell type.
-
Strong endoplasmic reticulum stressors
CautionISRIB loses its effect when the integrated stress response is strongly switched on, because phosphorylated eIF2 and ISRIB oppose each other's binding to eIF2B (PMID: 30674674, PMID: 33220178). Anything that drives the pathway hard, including severe endoplasmic reticulum stress, changes what ISRIB does.
-
Cisplatin
MonitorIn cell cultures and in mice, ISRIB reduced cisplatin-induced hair cell death and hearing loss without reducing cisplatin's effect on head and neck squamous cell carcinoma cells, unlike sodium thiosulfate (PMID: 39333235). This is an animal and cell finding with no human counterpart.

08Questions
Frequently asked questions
What is ISRIB?
ISRIB is a small molecule that blocks the integrated stress response, the signalling network that slows protein synthesis when cells are stressed. ISRIB came out of a screen of 106,281 compounds for inhibitors of PERK signalling run in Peter Walter's laboratory at the University of California, San Francisco, and was described in 2013 (PMID: 23741617). ISRIB has the molecular formula C22H24Cl2N2O4, a molecular weight of 451.3 g/mol and CAS number 1597403-47-8 for the trans isomer (PubChem CID 1011240).
How does ISRIB work?
ISRIB binds and activates eIF2B, the enzyme that recharges the translation initiation factor eIF2 with GTP, which is the step that the integrated stress response blocks (PMID: 25875391, PMID: 25858979). Cryo-electron microscopy showed ISRIB sitting in a cleft of the human eIF2B decamer, bridging a symmetry interface between two halves of the enzyme and favouring the fully assembled, active form (PMID: 29599213, PMID: 29599245). The effect of ISRIB depends on how strongly the pathway is switched on, because it works when phosphorylated eIF2 is present but loses its effect once phosphorylated eIF2 passes a threshold (PMID: 30674674, PMID: 33220178).
Has ISRIB been tested in humans?
No human study of ISRIB is registered. A ClinicalTrials.gov search on 25 October 2026 for ISRIB, trans-ISRIB and "integrated stress response inhibitor" as interventions returned no trials, and PubMed holds no human study of the compound. Chemists developing a clinical eIF2B activator wrote that the poor solubility of ISRIB limits its absorption and its advancement, which is the published reason newer molecules rather than ISRIB went into trials (PMID: 42153306).
Did the eIF2B activators that reached trials work?
Two eIF2B activators related to ISRIB have been tested in people with amyotrophic lateral sclerosis, and neither slowed the disease. The eIF2B activator DNL343 was given at 200 mg a day to 186 of the 249 participants randomised in a regimen of the HEALEY ALS Platform Trial, and that regimen gave a disease rate ratio of 1.04 with a 95 percent credible interval of 0.83 to 1.32, showing no slowing of progression (PMID: 42771346). The other eIF2B activator, fosigotifator or ABBV-CLS-7262, gave a disease rate ratio of 1.06 with a credible interval of 0.850 to 1.324 for its Dose 1 group in a 310-participant regimen of the same trial (ClinicalTrials.gov NCT05740813, results posted 14 November 2025). The two ALS results belong to DNL343 and fosigotifator and not to ISRIB, which has never been given to a person in a registered study.
What did ISRIB do to memory in animals?
In the original study, mice injected with ISRIB after each training trial found a hidden platform faster and showed better probe test performance, and mice given one injection after fear conditioning froze more at 24 hours but not at 1 hour (PMID: 23741617). In two mouse models of traumatic brain injury, ISRIB reversed hippocampal memory deficits and restored long-term potentiation even when given weeks after the injury (PMID: 28696288). In mice about 19 months old, three daily ISRIB injections improved water maze performance a week later and a working memory task weeks later (PMID: 33258451).
Are there studies where ISRIB did not work?
ISRIB has failed in two published mouse models of Alzheimer's disease. In the hAPP-J20 model of Alzheimer's disease, ISRIB at 0.25 and 2.5 mg/kg did not improve learning or memory in transgenic or non-transgenic mice despite confirmed drug exposure, and the authors could not detect the stress response marker ATF4 in brain tissue from either group (PMID: 27781164). In PS19 tau mice and APP amyloid mice, prolonged ISRIB did not improve behaviour or reduce tau pathology, and the authors found no evidence that the stress response was active in those models to begin with (PMID: 28721361). ISRIB also fails to suppress the integrated stress response once it is strongly activated (PMID: 30674674).
What is known about the safety of ISRIB?
ISRIB has no human safety data of any kind, because it has not been given to people in a registered study. In mice, daily ISRIB at 5 mg/kg was tolerated for 62 consecutive days by wild-type animals but had to be stopped after eight days in an APP amyloid cohort because of a significant rise in mortality, with seven ISRIB-treated mice dying in the second week of treatment, four of them non-transgenic (PMID: 28721361). In prion-infected mice, daily ISRIB left the pancreas structurally normal where a PERK kinase inhibitor destroyed exocrine tissue, but the treated mice still lost 20 percent of body weight by about 96 days (PMID: 25741597).
