Retatrutide Phase 3 Protocol Card
The TRIUMPH-1 and TRIUMPH-2 results on one page.
Retatrutide's phase 3 results were published in September 2026, TRIUMPH-1 in NEJM and TRIUMPH-2 in The Lancet. This card puts the trial titration and results by dose in one place.
01 / How the trials titrated
- 2mg weeks 1-4
- 4mg weeks 5-8
- 6mg weeks 9-12
- 9mg weeks 13-16
- 12mg week 17 onward
Every arm started at 2mg once weekly and stepped up every four weeks. Nobody in the 12mg group reached 12mg until around week 17. Lower-dose arms stopped climbing at their target (4mg or 9mg).
02 / Weight loss by dose at 80 weeks
| Dose | TRIUMPH-1, no diabetes | TRIUMPH-2, type 2 diabetes | Gap |
|---|---|---|---|
| 4mg | 19.0% | 12.7% | 6.3 pts |
| 9mg | 25.9% | 19.1% | 6.8 pts |
| 12mg | 28.3% | 20.8% | 7.5 pts |
| Placebo | - | 4.0% | - |
If you're insulin resistant, prediabetic or diabetic, TRIUMPH-2 is the trial that describes you. In the diabetic trial, going from 9mg to 12mg added only 1.7 points.
03 / The numbers that don't make headlines
11.3% - of the 12mg group quit due to side effects in TRIUMPH-1, versus 4.9% on placebo and 4.1% at 4mg
7.3% - dysesthesia (altered skin sensation) at 12mg in TRIUMPH-2, versus 0.7% on placebo. 4.5% at 4mg, 5.6% at 9mg
Up to 40% - of TRIUMPH-2 participants reached an A1C under 5.7%, the non-diabetic range. A1C fell by up to 1.6%
More than half - of the TRIUMPH-2 group on 12mg no longer met the BMI criteria for obesity by week 80
04 / What the trials measured
- Body weight (primary endpoint)
- Waist circumference
- A1C
- Fasting glucose
- Blood pressure
- Lipids
These are the markers the trials reported on, and the sensible ones to track against your own baseline.
05 / Where it stands
- Now: not approved in any country
- Q1 2027: Lilly files the BLA with the FDA
- +10-12 months: standard FDA review
- Late 2027: earliest realistic approval
