    Side Effects & Safety

    The two adverse events consistently reported in human tolerability work are headache and constipation, both slightly more intense and longer lasting in CYP2D6 poor metabolisers (PMID: 8363993). In the fibromyalgia literature, gastrointestinal symptoms including constipation increased at higher daily doses while the pain effect did not, producing a bell-shaped dose-response (PMID: 15515418, PMID: 15515419). In the schizophrenia trials the drug was generally well tolerated, although two of ten patients in the highest dose group of one 10-day study withdrew because of adverse effects (PMID: 22952075). Animal toxicology showed no specific organ toxicity beyond slowed body weight gain (PMID: 8363993).

    Contraindications

    Sourced and mechanism-based: metabolism depends on CYP2D6, so ultrarapid metabolisers get lower exposure and reduced antiemetic effect while poor metabolisers get higher exposure and more adverse effects (PMID: 12065557, PMID: 11736884), the basis of the Clinical Pharmacogenetics Implementation Consortium guideline for CYP2D6 genotype and 5-HT3 receptor antagonists (PMID: 41979467).
