Selegiline, also called L-deprenyl or (-)-deprenyl, is an irreversible inhibitor of the enzyme monoamine oxidase B (MAO-B), created in the 1960s by Joseph Knoll of Semmelweis University in Budapest and approved by the FDA in 1989 (labeled as an add-on to levodopa in Parkinson's disease) and in 2006 as the Emsam skin patch for major depressive disorder in adults.

01Overview
What Selegiline is
In the DATATOP trial of 800 people with early Parkinson's disease, selegiline delayed the need for levodopa by a median of about nine months, but a reanalysis judged the effect transient and likely symptomatic, early selegiline did not reduce later levodopa complications, and deaths did not differ (PMID: 8417384, PMID: 1471871, PMID: 8572664, PMID: 9506548). Knoll reported in 1988 that rats given selegiline from the end of their second year lived much longer than saline-treated rats; later studies in rats, mice, hamsters and dogs gave mixed results, a 2025 meta-analysis of 22 rodent experiments found a moderate average lifespan increase with substantial heterogeneity, and randomized trials in Parkinson's disease found no survival benefit in people (PMID: 3147347, PMID: 8247225, PMID: 9263197, PMID: 40816452, PMID: 15310558). The US labels warn of dangerous reactions with meperidine and serotonergic antidepressants and of blood pressure crises after tyramine-rich foods, a risk that grows as selegiline loses its selectivity for MAO-B at higher doses (US prescribing information for selegiline hydrochloride tablets; US prescribing information for Emsam).
How it works
MAO-B inhibition
Selegiline is an irreversible inhibitor of monoamine oxidase, a mitochondrial enzyme that breaks down dopamine, noradrenaline, serotonin and dietary amines; MAO converts selegiline into a reactive form that binds the enzyme permanently (suicide inhibition), and selegiline's greater affinity for the B form makes it a selective MAO-B inhibitor at the labeled oral dose (US prescribing information for selegiline hydrochloride tablets). The label states that MAO in the human gut is mainly type A and that most brain MAO is type B, and that selegiline's benefit in Parkinson's disease is thought to come mainly from blocking dopamine breakdown, although the mechanism is not fully understood (US prescribing information for selegiline hydrochloride tablets).

Tyramine and the cheese reaction (human studies)
In 1978, 10 volunteers (4 healthy and 6 with Parkinson's disease) whose platelet MAO-B was fully inhibited by selegiline had no blood pressure surge (cheese effect) after oral tyramine in amounts larger than a normal diet supplies, and human intestinal lining was shown to contain mainly MAO-A (PMID: 96466). Selectivity falls as the dose rises: in healthy volunteers, selegiline at 30 mg a day made people 2 to 4 times more sensitive to oral tyramine, with signs of MAO-A inhibition (PMID: 3145523), and a 1989 study concluded that selegiline does not selectively inhibit MAO-B at 20 mg a day and above (PMID: 2510962). The oral label states that selectivity may not be absolute even at the labeled dose and that selegiline may become non-selective at about 30 to 40 mg a day (US prescribing information for selegiline hydrochloride tablets). The Emsam label states that at antidepressant doses selegiline inhibits both MAO-A and MAO-B, and that selegiline absorbed through the skin does not undergo the extensive first-pass metabolism seen after oral doses (US prescribing information for Emsam; US prescribing information for selegiline hydrochloride tablets).
Recovery after stopping (human PET study)
Because the inhibition is irreversible, MAO-B activity returns only as new enzyme is made: in a PET study at Brookhaven National Laboratory, brain MAO-B was resynthesized with a half-time of about 40 days after selegiline was stopped (PMID: 7839316). Platelet MAO-B returns to normal within 5 to 7 days of stopping (US prescribing information for selegiline hydrochloride tablets).
Metabolites
Selegiline is broken down to N-desmethylselegiline, which also inhibits MAO-B, and to L-methamphetamine and L-amphetamine (US prescribing information for selegiline hydrochloride tablets). In a 1989 study, 55 percent of a selegiline dose was excreted in urine as amphetamine and methamphetamine (PMID: 2510962). The Emsam label states that these metabolites are the levorotatory (R) forms, that no conversion to the dextrorotatory forms occurs, and that in human liver microsomes CYP2B6, CYP2C9, CYP3A4 and CYP3A5 were the main enzymes forming R(-)-methamphetamine from selegiline (US prescribing information for Emsam).
The enhancer hypothesis (Knoll's group)
Joseph Knoll's group at Semmelweis University later proposed that selegiline, a synthetic derivative of beta-phenylethylamine, also raises the activity of catecholamine-releasing neurons (a catecholaminergic activity enhancer effect) by a mechanism unrelated to MAO-B inhibition, and this line of work led to BPAP, a more potent tryptamine-derived enhancer (PMID: 27480491). The group's proposal to use selegiline against brain aging rests on two longevity studies in male rats (PMID: 27480491).
Pharmacokinetics
By mouth, selegiline undergoes extensive first-pass metabolism, food raises its bioavailability 3 to 4 fold, and the elimination half-life is about 2 hours after one dose and about 10 hours at steady state (US prescribing information for selegiline hydrochloride tablets). From the skin patch, 25 to 30 percent of the selegiline content is absorbed over 24 hours on average, exposure to selegiline is higher and exposure to its metabolites lower than by mouth, about 90 percent is bound to plasma proteins, and after intravenous dosing the half-lives of selegiline and its three main metabolites ranged from 18 to 25 hours (US prescribing information for Emsam).

Who should not use it
Contraindications
Labeled contraindications for oral selegiline are known hypersensitivity to selegiline and use with meperidine, a contraindication that the label notes is often extended to other opioids (US prescribing information for selegiline hydrochloride tablets).
Read all the contraindications and cautionsShow less
Labeled contraindications for Emsam are use with selective serotonin reuptake inhibitors, serotonin and norepinephrine reuptake inhibitors, clomipramine, imipramine, meperidine, tramadol, methadone, pentazocine, propoxyphene, dextromethorphan or carbamazepine; children under 12; and pheochromocytoma (US prescribing information for Emsam). The label requires at least 2 weeks after stopping Emsam before starting a contraindicated drug, and at least 5 weeks after stopping fluoxetine before starting Emsam (US prescribing information for Emsam).
Doses above the labeled range are a concern: the oral label warns that selectivity for MAO-B is reduced as the daily dose rises, with the risks of non-selective MAO inhibition (US prescribing information for selegiline hydrochloride tablets), and healthy volunteers taking 30 mg a day became 2 to 4 times more sensitive to tyramine (PMID: 3145523).
The Emsam label requires a low-tyramine diet at the 9 and 12 mg per 24 hours strengths, from the first day until 2 weeks after a dose reduction or stop, and lists aged cheeses, air-dried, aged or fermented meats, tap beer, concentrated yeast extract, sauerkraut and most soy products among the foods to avoid (US prescribing information for Emsam).
Antidepressants, Emsam included, can trigger mania in people with bipolar disorder, and the label calls for screening before treatment (US prescribing information for Emsam).
The Emsam label advises against exposing the patch site to heating pads, saunas, hot tubs or prolonged direct sunlight, because heat may increase absorption (US prescribing information for Emsam).
There are no adequate studies in pregnant women; in rabbits the highest dose increased fetal loss, and in rats high doses increased stillbirths and reduced pup survival (US prescribing information for selegiline hydrochloride tablets).
Interactions
7 pairings in our record: 4 to avoid, 2 with a caution and 1 to monitor.
Meperidine and other opioids
AvoidSelegiline is contraindicated with meperidine; stupor, muscle rigidity, severe agitation, high temperature and deaths have been reported with the combination, and the label notes that the contraindication is often extended to other opioids (US prescribing information for selegiline hydrochloride tablets). The Emsam label also contraindicates tramadol, methadone, pentazocine and propoxyphene (US prescribing information for Emsam).
SSRIs, SNRIs and tricyclic antidepressants
AvoidSevere reactions, some fatal, have been reported with selegiline plus tricyclic antidepressants or selective serotonin reuptake inhibitors, and the oral label calls for at least 14 days between stopping selegiline and starting these drugs and at least 5 weeks after stopping fluoxetine (US prescribing information for selegiline hydrochloride tablets). Emsam is contraindicated with SSRIs, SNRIs, clomipramine and imipramine because of serotonin syndrome (US prescribing information for Emsam).
Dextromethorphan
AvoidEmsam is contraindicated with the cough suppressant dextromethorphan because of a risk of serotonin syndrome (US prescribing information for Emsam).
Carbamazepine
AvoidCarbamazepine significantly raised selegiline levels and is contraindicated with Emsam because of a possible higher risk of hypertensive crisis (US prescribing information for Emsam).
Tyramine-rich foods
CautionThe Emsam label requires a low-tyramine diet at the 9 and 12 mg per 24 hours strengths but not at 6 mg per 24 hours (US prescribing information for Emsam), and the oral label reports rare hypertensive reactions with tyramine-containing foods at the labeled dose (US prescribing information for selegiline hydrochloride tablets).
Sympathomimetic amines, amphetamines and buspirone
CautionThe Emsam label calls for blood pressure monitoring with buspirone, amphetamines and cold or weight-loss products containing pseudoephedrine, phenylephrine, phenylpropanolamine or ephedrine (US prescribing information for Emsam), and one hypertensive crisis was reported with oral selegiline plus ephedrine (US prescribing information for selegiline hydrochloride tablets).
Levodopa
MonitorSelegiline can worsen levodopa side effects (US prescribing information for selegiline hydrochloride tablets), and in the DATATOP extension people who stayed on selegiline with levodopa had more dyskinesias but less freezing of gait (PMID: 12112107).

- Half-life
- About 2 hours after a single oral dose and about 10 hours at steady state (US prescribing information for selegiline hydrochloride tablets); the Emsam label gives no half-life for the patch itself and reports 18 to 25 hours for selegiline and its three main metabolites after an intravenous dose (US prescribing information for Emsam). 08Questions
Frequently asked questions
What is selegiline?
Selegiline, also known as L-deprenyl, is an irreversible inhibitor of monoamine oxidase B that Joseph Knoll created in Budapest in the 1960s (PMID: 27480491; US prescribing information for selegiline hydrochloride tablets). Selegiline is a derivative of phenethylamine with the formula C13H17N and a molecular weight of 187.28 g/mol in PubChem (CID 26757; US prescribing information for selegiline hydrochloride tablets). Selegiline is approved in the US as tablets and capsules for use alongside levodopa/carbidopa in Parkinson's disease when the response to levodopa is deteriorating, and as the Emsam skin patch for major depressive disorder in adults (US prescribing information for selegiline hydrochloride tablets; US prescribing information for Emsam).
What did the DATATOP trial show about selegiline?
In DATATOP, 800 people with early, untreated Parkinson's disease were randomized to selegiline, tocopherol, both or placebo, and selegiline delayed the need for levodopa (hazard ratio 0.50, median delay about nine months) (PMID: 8417384). Parkinson's ratings improved in the first three months on selegiline and worsened after selegiline was withdrawn, and a 1992 reanalysis judged the selegiline effect transient and likely symptomatic (PMID: 8417384, PMID: 1471871). Earlier selegiline did not reduce later levodopa complications, and deaths over 8.2 years did not differ by selegiline assignment (PMID: 8572664, PMID: 9506548).
Does selegiline extend lifespan?
In Knoll's 1988 study, male rats given selegiline from the end of their second year lived on average 197.98 weeks against 147.05 weeks on saline (PMID: 3147347). Later selegiline studies were mixed, with no survival effect in aged mice at the US National Institute on Aging and no clear effect in middle-aged Fischer rats (PMID: 8247225, PMID: 9263197). A 2025 meta-analysis of 22 rodent experiments found a moderate average lifespan increase with selegiline but substantial heterogeneity (PMID: 40816452). Randomized trials of selegiline in Parkinson's disease found no longer survival in people (PMID: 9506548, PMID: 15310558).
Why do selegiline labels warn about tyramine and cheese?
Selegiline blocks MAO-B selectively only at low doses, and at higher doses selegiline also blocks MAO-A, the gut enzyme that breaks down tyramine from foods such as aged cheese (US prescribing information for selegiline hydrochloride tablets; PMID: 3145523). The oral selegiline label reports rare hypertensive reactions with tyramine-containing foods even at the labeled dose (US prescribing information for selegiline hydrochloride tablets). The Emsam label requires a low-tyramine diet at the 9 and 12 mg per 24 hours strengths of selegiline but not at 6 mg per 24 hours (US prescribing information for Emsam).
Which drug combinations do the selegiline labels contraindicate or warn against?
Oral selegiline's label contraindicates meperidine and warns against combining selegiline with tricyclic antidepressants and SSRIs after severe and sometimes fatal reactions (US prescribing information for selegiline hydrochloride tablets). The Emsam label contraindicates selegiline with SSRIs, SNRIs, clomipramine, imipramine, tramadol, methadone, pentazocine, propoxyphene, dextromethorphan and carbamazepine (US prescribing information for Emsam). The Emsam label also calls for blood pressure monitoring when selegiline is used with buspirone, amphetamines or decongestants such as pseudoephedrine (US prescribing information for Emsam).
How is selegiline related to BPAP?
Joseph Knoll's group proposed that selegiline, apart from blocking MAO-B, raises the activity of catecholamine-releasing neurons by a separate enhancer effect, and the same group developed BPAP as a more potent enhancer derived from tryptamine (PMID: 27480491). In a 2016 rat study by Knoll and Miklya, BPAP extended lifespan more potently than selegiline at low doses (PMID: 27777099).
What are the common side effects of selegiline?
In the Parkinson's disease trial described in the oral selegiline label, nausea, dizziness or fainting, abdominal pain, confusion, hallucinations and dry mouth were more frequent than on placebo (US prescribing information for selegiline hydrochloride tablets). With the Emsam patch, the most common side effects of selegiline that were more frequent than on placebo were application site reactions (24 against 12 percent), headache (18 against 17 percent), insomnia (12 against 7 percent) and diarrhea (9 against 7 percent) (US prescribing information for Emsam).
