    Keep continuous use time-limited (the GAD RCT ran 6 weeks [PMID:23635869]) and take periodic breaks. Re-check ALT/AST if using regularly beyond a few weeks, and stop immediately at any sign of liver trouble (dark urine, jaundice, right-upper-quadrant pain, unusual fatigue).

    Advanced

    Experienced users sometimes push total daily kavalactone intake toward 300-500 mg (from kavain plus whole-kava sources) for acute situational anxiety, but there is NO controlled human evidence that isolated kavain is safe or effective at these doses, and higher dose and longer duration are documented risk factors for kava hepatotoxicity [PMID:20720265]. This is not a recommendation.

    If used at all: single-day or short-run situational dosing rather than chronic daily use; strict abstinence from alcohol and other liver-stressing agents; and lab monitoring. Chronic heavy kava use is also associated with a reversible scaly skin rash (kava dermopathy). There is no established benefit to daily high-dose use, and the risk/benefit balance worsens the higher and longer you go.

Rare but serious: hepatotoxicity ranging from asymptomatic liver-enzyme elevation to acute liver injury and, rarely, liver failure requiring transplant [PMID:20720265][PMID:27092496][PMID:34307603]. Stop immediately and seek care for jaundice, dark urine, pale stools, right-upper-quadrant pain, nausea or unusual fatigue.

    Contraindications

    Do not use with: existing liver disease or elevated liver enzymes; alcohol; other hepatotoxic drugs or supplements; pregnancy or breastfeeding. Avoid before driving or operating machinery, and before surgery (CNS-depressant and possible additive effects with anesthesia). Caution or avoid with benzodiazepines, barbiturates, opioids, Z-drugs and other CNS depressants (additive sedation). Because kava/kavain has dopamine D2 antagonist activity [PMID:11458444], avoid in Parkinson's disease or with dopaminergic therapy. Not for use under 18. Discuss with a clinician before use if you take any prescription medication, especially drugs metabolized by liver CYP450 enzymes.
