Cabergoline is a long-acting, ergot-derived dopamine D2 receptor agonist developed by Farmitalia Carlo Erba in Milan under the code FCE 21336 and approved by the FDA as Dostinex on 23 December 1996 for hyperprolactinemic disorders, either idiopathic or due to pituitary adenomas.

    01Overview

    What Cabergoline is

In a 459-woman trial, cabergoline normalized prolactin in 83 percent against 59 percent with bromocriptine, with fewer women stopping for intolerance (3 against 12 percent), and in 51 men with prolactinomas it normalized prolactin in about three quarters and shrank the tumors (PMID: 7915824, PMID: 15070934). Cabergoline is a potent agonist at serotonin 5-HT2B receptors in laboratory tissue, and at the high doses once used for Parkinson's disease it was linked to clinically important heart valve regurgitation, while studies at the lower doses used for high prolactin found more tricuspid regurgitation on echocardiography, none diagnosed from symptoms, and no excess of valve surgery or heart failure (PMID: 15862804, PMID: 17202454, PMID: 30215804, PMID: 33247916). For men taking both cabergoline and testosterone, the published data come from an uncontrolled clinic review of men with delayed or absent orgasm, 65.6 percent of whom were on testosterone therapy, and from studies of men with prolactinomas who were given testosterone replacement when their testosterone stayed low on cabergoline (PMID: 26944776, PMID: 15070934, PMID: 25592453).

    How it works

    Dopamine D2 agonism and prolactin

Cabergoline is an ergot derivative and dopamine receptor agonist with high affinity for D2 receptors; in laboratory studies it directly inhibited prolactin secretion by rat pituitary lactotroph cells, and receptor-binding studies showed low affinity for dopamine D1, alpha1- and alpha2-adrenergic, and 5-HT1 and 5-HT2 serotonin receptors (US prescribing information for cabergoline tablets). In healthy men given single doses, prolactin fell within 3 hours and stayed suppressed for up to 168 hours after the higher doses tested (PMID: 3580251). In 12 healthy men given three single doses of increasing size several weeks apart, every dose suppressed prolactin below 1.0 microgram per liter in every subject, blood pressure fell progressively, and one man could not take the highest dose because of symptomatic low blood pressure after the middle dose (PMID: 7883840).

Serotonin 5-HT2B receptors and heart valves

Agonism at serotonin 5-HT2B receptors is considered the most likely cause of the fibrotic valve disease seen with dopamine agonists such as pergolide and cabergoline (PMID: 21518772). In pig pulmonary artery tissue, cabergoline and pergolide were potent full agonists at 5-HT2B receptors, bromocriptine was a partial agonist, and lisuride and terguride blocked the receptor, so this property is not shared by all ergot-derived drugs (PMID: 15862804). The label's receptor-binding summary, by contrast, lists low affinity for 5-HT2 receptors (US prescribing information for cabergoline tablets). A 2011 study traced cabergoline's agonism at 5-HT2B and 5-HT2A receptors to the allyl group on its N6 nitrogen and found that cabergoline stimulated growth signaling and extracellular matrix production in pig heart valve cells through 5-HT2A receptors, though the authors judged its potency at native 5-HT2A receptors too moderate for these to be the main target in valve disease (PMID: 21518772).

    Pharmacokinetics

Peak plasma levels of cabergoline come 2 to 3 hours after an oral dose, about 40 percent is bound to plasma proteins, metabolism is mainly by hydrolysis with minimal cytochrome P450 involvement, and less than 4 percent is excreted unchanged in urine (PMID: 12844325). The label gives an elimination half-life of 63 to 69 hours estimated from urinary data in 12 healthy subjects, and a 2003 review gives 63 to 109 hours (US prescribing information for cabergoline tablets; PMID: 12844325). Cabergoline exposure (AUC) rose 1.5-fold in moderate and 5.6-fold in severe liver impairment, while moderate to severe kidney impairment did not change its pharmacokinetics (US prescribing information for cabergoline tablets). In healthy men, the CYP3A4 and P-glycoprotein inhibitor clarithromycin raised mean plasma cabergoline 2.6-fold (PMID: 16410677).

    08Questions

    Frequently asked questions

    What is cabergoline?

Cabergoline is an ergot-derived dopamine agonist with high affinity for D2 receptors, approved by the FDA in 1996 as Dostinex for hyperprolactinemic disorders, either idiopathic or due to pituitary adenomas (US prescribing information for cabergoline tablets; Drugs@FDA NDA 020664). Cabergoline lowers prolactin for days after one dose, and in healthy men prolactin stayed suppressed for up to 168 hours after single doses (PMID: 3580251). Cabergoline is sold in the US as generic prescription tablets, and the Dostinex brand is listed as discontinued for reasons other than safety or effectiveness (Drugs@FDA).

    Does cabergoline damage heart valves?

At the high doses once used for Parkinson's disease, cabergoline was linked to clinically important valve regurgitation, found in 28.6 percent of 49 cabergoline-treated Parkinson's patients against 5.6 percent of 90 control subjects in one echocardiography study (PMID: 17202454, PMID: 30215804). At the lower doses used for high prolactin, a 2018 meta-analysis found more tricuspid regurgitation with cabergoline than in controls (odds ratio 3.74), none diagnosed from symptoms (PMID: 30215804). In a 2021 study of 646 patients taking cabergoline for prolactinoma, rates of valve surgery or heart failure did not differ from matched controls (PMID: 33247916). The current cabergoline label contraindicates the drug in people with a history of heart valve disease and calls for echocardiograms before and during cabergoline treatment (US prescribing information for cabergoline tablets).

What do studies report about cabergoline in men taking testosterone?

A search of PubMed and ClinicalTrials.gov on 27 October 2026 found no randomized trial of cabergoline in men on testosterone therapy. In a retrospective review of 131 men given cabergoline off-label for delayed or absent orgasm, 65.6 percent were also on testosterone therapy and 66.4 percent reported subjective improvement, with no placebo group (PMID: 26944776). In studies of men with prolactinomas, men whose testosterone stayed low on cabergoline were given testosterone or gonadotropin replacement (PMID: 15070934, PMID: 25592453). A study of 308 patients with prolactinoma on dopamine agonists such as cabergoline found hypersexuality and other impulse control disorders more common in men (PMID: 30848825).

What are the most common side effects of cabergoline?

In a 4-week placebo-controlled trial, the most frequent cabergoline side effects were nausea (27 percent against 20 percent on placebo), headache (26 against 25 percent) and dizziness (15 against 5 percent) (US prescribing information for cabergoline tablets). Low blood pressure on standing affected 4 percent of women on cabergoline and none on placebo in the same trial (US prescribing information for cabergoline tablets). In a 459-woman trial, 3 percent of women stopped cabergoline because of intolerance against 12 percent on bromocriptine (PMID: 7915824).

Can cabergoline cause impulse control problems?

The cabergoline label warns of intense urges to gamble, spend money, binge eat or have sex in people taking drugs that increase dopamine activity (US prescribing information for cabergoline tablets). In 308 patients with prolactinoma on dopamine agonists such as cabergoline, 17 percent screened positive for an impulse control disorder, and hypersexuality was more common in men (PMID: 30848825). A 2025 meta-analysis found impulse control disorders 1.71 times as prevalent with dopamine agonists, the class that includes cabergoline, in patients with prolactinoma, mainly hypersexuality and punding (PMID: 40480033).

How long does cabergoline stay in the body?

The cabergoline label gives an elimination half-life of 63 to 69 hours, estimated from urinary data in 12 healthy subjects (US prescribing information for cabergoline tablets). A 2003 review gives a range of 63 to 109 hours for cabergoline (PMID: 12844325). In healthy men, single doses of cabergoline kept prolactin suppressed for up to 168 hours (PMID: 3580251).

Is cabergoline used to stop breast milk after childbirth?

In a 1991 European trial of 272 women, one dose of cabergoline prevented lactation at least as well as 14 days of bromocriptine, with less rebound breast activity (PMID: 1676318). The current US cabergoline label advises against using cabergoline to inhibit or suppress breast milk after childbirth, because bromocriptine used this way has been associated with high blood pressure, stroke, heart attack, seizures and death (US prescribing information for cabergoline tablets).
