Telmisartan is an angiotensin II receptor blocker that lowers blood pressure by blocking the angiotensin II type 1 (AT1) receptor, approved by the FDA on 10 November 1998 and sold under the brand name Micardis (Drugs@FDA NDA 020850; US prescribing information for telmisartan tablets).

01Overview
What Telmisartan is
The FDA added a second telmisartan indication in 2009: reducing the risk of heart attack, stroke or death from cardiovascular causes in people 55 or older at high cardiovascular risk who cannot take ACE inhibitors (Drugs@FDA NDA 020850, supplement 25 approved 16 October 2009). Interest in telmisartan outside cardiology comes from a 2004 finding that it also acts as a partial agonist of the nuclear receptor PPAR-gamma, which no other marketed angiotensin receptor blocker did at concentrations reached with ordinary oral doses; that work was done in cells and in rats fed a high-fat, high-carbohydrate diet (PMID: 15007034, PMID: 15117841). In the two large outcome trials, ONTARGET in 25,620 patients and TRANSCEND in 5,926 patients, telmisartan performed about as well as ramipril and did not beat placebo on its primary endpoint (PMID: 18378520, PMID: 18757085).
How it works
Blocking the angiotensin II receptor
Angiotensin II is the main pressure-raising product of the renin-angiotensin system. It causes blood vessels to narrow, stimulates aldosterone release, stimulates the heart and increases sodium reabsorption in the kidney. Telmisartan blocks the binding of angiotensin II to the AT1 receptor in tissues such as vascular smooth muscle and the adrenal gland, with more than 3,000 times greater affinity for AT1 than for the AT2 receptor, and it does not inhibit the angiotensin-converting enzyme, so it does not affect the response to bradykinin (US prescribing information for telmisartan tablets). In healthy volunteers, telmisartan at 80 mg blocked about 90 percent of the blood pressure response to infused angiotensin II at peak plasma levels, with about 40 percent of that blockade still present 24 hours later (US prescribing information for telmisartan tablets).

Partial PPAR-gamma activity, in cells and rats
PPAR-gamma (peroxisome proliferator-activated receptor gamma) is the nuclear receptor targeted by the thiazolidinedione diabetes drugs. A 2004 report from Theodore Kurtz's group identified telmisartan as a partial agonist of PPAR-gamma that changed the expression of PPAR-gamma target genes and lowered glucose, insulin and triglycerides in rats fed a high-fat, high-carbohydrate diet; none of the other angiotensin receptor blockers on the market activated PPAR-gamma when tested at concentrations typically reached in plasma with conventional oral dosing (PMID: 15007034). A group at the Charite in Berlin reported the same year that telmisartan and irbesartan at 10 micromolar promoted PPAR-gamma-dependent differentiation of mouse 3T3-L1 fat cells, that telmisartan raised PPAR-gamma transcriptional activity 2.6-fold against 5.2-fold for pioglitazone, and that the effect persisted in a cell line lacking the AT1 receptor, so it is separate from receptor blockade (PMID: 15117841). Both results are cell and rodent data.
What human studies of insulin sensitivity show
Meta-analyses of randomized trials that compared telmisartan with other angiotensin receptor blockers report small advantages for telmisartan on insulin measures: 21 trials in 1,679 patients found a mean difference in HOMA-IR of -0.23 (PMID: 29557807), 33 trials in 2,033 hypertensive patients found insulin levels 10.9 percent lower and HOMA-IR 15.9 percent lower, with smaller differences in the 8 double-blind trials (PMID: 25151319), and 10 trials in 546 people with metabolic syndrome found lower fasting glucose, insulin and glycated hemoglobin and higher adiponectin (PMID: 23523138). Trials that measured insulin action directly with a hyperinsulinemic euglycemic clamp are less supportive. In 12 men with impaired fasting glucose given telmisartan at 160 mg a day for 3 weeks in a crossover trial, their fasting glucose fell but insulin sensitivity and substrate use did not change (PMID: 20630944), the same group found no change in muscle fat content in 10 similar participants (PMID: 26523653), and a 138-person placebo-controlled trial in overweight adults without hypertension or diabetes found no difference after 16 weeks in an insulin sensitivity index calculated from oral glucose tolerance testing, with clamp measurements in a subset of those participants (PMID: 20883237).
PPAR-delta and AMPK in mouse studies
A group at the Third Military Medical University in Chongqing reported that telmisartan raised PPAR-delta expression in fat cell precursors and prevented high-fat-diet obesity in wild-type mice and in hypertensive rats without changing food intake, and that the effect was absent in PPAR-delta knockout mice (PMID: 20176998). The same group reported that telmisartan raised phosphorylated AMPK in cultured muscle cells and in mouse skeletal muscle, increased running endurance and post-exercise oxygen consumption, and increased slow-twitch fibres, again with no effect in PPAR-delta-deficient mice (PMID: 20477906). These are mouse and rat results, and the doses used are not human doses.
What the aging claims rest on
Mice in which the gene for the AT1a receptor is deleted live longer than wild-type mice, develop less cardiac and vascular injury and show more mitochondria and higher Nampt and Sirt3 in the kidney; this was a genetic deletion at the Mario Negri Institute in Bergamo, not telmisartan treatment (PMID: 19197138). In cultured human endothelial cells passaged to senescence, telmisartan raised DDAH activity, lowered asymmetric dimethylarginine, raised nitric oxide metabolites and delayed senescence, and blocking PPAR-gamma removed the effect (PMID: 18250362). The National Institute on Aging Interventions Testing Program tested atorvastatin combined with telmisartan in genetically heterogeneous UM-HET3 mice and found no increase in lifespan in either sex; females on that combination weighed less than controls from 12 months (PMID: 41843349).
Pharmacokinetics
Telmisartan reaches peak plasma concentrations 0.5 to 1 hour after an oral dose, is more than 99.5 percent bound to plasma proteins, has a volume of distribution of about 500 litres and a terminal elimination half-life of about 24 hours, and more than 97 percent of a dose leaves the body unchanged in feces through biliary excretion (US prescribing information for telmisartan tablets). Cytochrome P450 enzymes are not involved in its metabolism, the only metabolite identified in human plasma is an inactive acyl glucuronide, and in laboratory tests telmisartan had no effect on cytochrome P450 enzymes except some inhibition of CYP2C19 (US prescribing information for telmisartan tablets). Plasma concentrations are 2 to 3 times higher in women than in men without a change in blood pressure response, and bioavailability approaches 100 percent in liver impairment (US prescribing information for telmisartan tablets).

Who should not use it
Contraindications
Telmisartan carries a boxed warning for fetal toxicity. Use of drugs acting on the renin-angiotensin system in the second and third trimesters reduces fetal kidney function and raises fetal and neonatal illness and death, and the label says to stop telmisartan as soon as pregnancy is detected (US prescribing information for telmisartan tablets).
Read all the contraindications and cautionsShow less
Telmisartan is contraindicated in anyone with known hypersensitivity, such as anaphylaxis or angioedema, to telmisartan or any component of the product (US prescribing information for telmisartan tablets).
The label contraindicates giving aliskiren together with telmisartan in patients with diabetes, and says to avoid the combination in renal impairment with a glomerular filtration rate below 60 mL/min/1.73 m2 (US prescribing information for telmisartan tablets).
Dual blockade of the renin-angiotensin system with an angiotensin receptor blocker plus an ACE inhibitor or aliskiren raises the risk of hypotension, hyperkalemia and changes in kidney function, and the ONTARGET combination group had more renal dysfunction than either drug alone (US prescribing information for telmisartan tablets; PMID: 18378520, PMID: 18707986).
In volume or salt depletion, symptomatic hypotension can follow the first doses in people with an activated renin-angiotensin system, for example those on high doses of diuretics, and the label says to correct the depletion first or start under close supervision (US prescribing information for telmisartan tablets).
Potassium can rise (hyperkalemia), particularly in advanced renal impairment, heart failure, renal replacement therapy, or with potassium supplements, potassium-sparing diuretics or potassium-containing salt substitutes (US prescribing information for telmisartan tablets).
Most telmisartan leaves the body in bile, so clearance is reduced and bioavailability approaches 100 percent in biliary obstruction or hepatic insufficiency; the label also warns of oliguria, progressive azotemia and rarely acute renal failure in people whose kidney function depends on the renin-angiotensin-aldosterone system (US prescribing information for telmisartan tablets).
The label advises a nursing woman not to breastfeed during treatment with telmisartan, because of the potential for hypotension, hyperkalemia and renal impairment in the infant; telmisartan is present in the milk of lactating rats (US prescribing information for telmisartan tablets).
Safety and effectiveness in children (patients under 18) have not been established and telmisartan pharmacokinetics have not been studied in that age group (US prescribing information for telmisartan tablets).
Interactions
7 pairings in our record: 2 to avoid, 1 with a caution and 4 to monitor.
Aliskiren
AvoidThe label contraindicates giving aliskiren with telmisartan in patients with diabetes and says to avoid the combination when the glomerular filtration rate is below 60 mL/min/1.73 m2 (US prescribing information for telmisartan tablets).
ACE inhibitors (ramipril and others)
AvoidIn ONTARGET, telmisartan added to ramipril gave no extra benefit and more hypotension, syncope and renal dysfunction, and the label says to avoid combined use of renin-angiotensin system inhibitors; co-administration also raised ramipril exposure about twofold in healthy subjects (PMID: 18378520, PMID: 18707986; US prescribing information for telmisartan tablets).
CYP2C19 substrates
CautionTelmisartan is not metabolized by cytochrome P450 enzymes and showed no clinically significant interaction with acetaminophen, amlodipine, glyburide, simvastatin, hydrochlorothiazide, warfarin or ibuprofen, but it inhibited CYP2C19 in laboratory tests, so the label flags possible interference with drugs cleared by CYP2C19 (US prescribing information for telmisartan tablets).
NSAIDs
MonitorNSAIDs, selective COX-2 inhibitors among them, can reduce the blood pressure effect of telmisartan and, in elderly, volume-depleted or renally impaired patients, can worsen kidney function; the label calls for periodic monitoring of renal function (US prescribing information for telmisartan tablets).
Lithium
MonitorReversible rises in serum lithium and lithium toxicity have been reported with angiotensin II receptor antagonists including telmisartan, and the label says to monitor lithium levels during combined use (US prescribing information for telmisartan tablets).
Digoxin
MonitorTelmisartan at 120 mg a day raised steady-state digoxin area under the curve by 22 percent and peak concentration by 50 percent in 12 healthy men, and the label says to monitor digoxin levels when telmisartan is started, changed or stopped (PMID: 11185636; US prescribing information for telmisartan tablets).
Potassium supplements and potassium-sparing diuretics
MonitorPotassium supplements, potassium-sparing diuretics and potassium-containing salt substitutes add to the risk of hyperkalemia with an angiotensin receptor blocker, and the label suggests periodic electrolyte measurements in patients at risk (US prescribing information for telmisartan tablets).

08Questions
Frequently asked questions
What is telmisartan?
Telmisartan is an angiotensin II receptor blocker that lowers blood pressure by blocking the angiotensin II type 1 receptor, with more than 3,000 times greater affinity for that receptor than for the AT2 receptor (US prescribing information for telmisartan tablets). Telmisartan has been an FDA-approved prescription drug since 10 November 1998 and was sold as Micardis (Drugs@FDA NDA 020850). Generic telmisartan tablets from more than a dozen manufacturers are the prescription products on the US market today (Drugs@FDA).
What is telmisartan approved for?
Telmisartan is approved for the treatment of hypertension, to lower blood pressure (Drugs@FDA NDA 020850). A second telmisartan indication, added by a supplement approved on 16 October 2009, covers reduction of the risk of myocardial infarction, stroke or death from cardiovascular causes in patients 55 or older at high cardiovascular risk who are unable to take ACE inhibitors (Drugs@FDA NDA 020850, supplement 25). The telmisartan label carries a boxed warning for fetal toxicity and says to stop the drug as soon as pregnancy is detected (US prescribing information for telmisartan tablets).
Why is telmisartan discussed as a longevity compound?
Telmisartan is the one marketed angiotensin receptor blocker found to act as a partial agonist of PPAR-gamma at concentrations reached with ordinary oral doses, which links it to the nuclear receptor targeted by insulin-sensitizing diabetes drugs (PMID: 15007034, PMID: 15117841). Deleting the gene for the AT1a receptor in mice extends their lifespan and reduces oxidative damage, which is a genetic experiment and not a test of telmisartan (PMID: 19197138). When the National Institute on Aging Interventions Testing Program tested atorvastatin combined with telmisartan in genetically heterogeneous mice, lifespan did not increase in either sex (PMID: 41843349). No human trial has tested telmisartan for lifespan or for any aging endpoint.
What did ONTARGET and TRANSCEND find?
ONTARGET randomized 25,620 patients at high cardiovascular risk to telmisartan, ramipril or both, and at a median 56 months the primary composite endpoint occurred in 16.7 percent on telmisartan against 16.5 percent on ramipril (relative risk 1.01, 95% CI 0.94 to 1.09), which the authors read as equivalence (PMID: 18378520). The combination of telmisartan and ramipril in ONTARGET brought no extra benefit and more hypotension, syncope and renal dysfunction (PMID: 18378520, PMID: 18707986). TRANSCEND randomized 5,926 patients who could not tolerate ACE inhibitors to telmisartan or placebo and missed its primary endpoint (15.7 against 17.0 percent, hazard ratio 0.92, 95% CI 0.81 to 1.05, p=0.216), while a secondary composite of cardiovascular death, myocardial infarction or stroke reached p=0.048 before adjustment and p=0.068 after (PMID: 18757085).
Does telmisartan improve insulin sensitivity in people?
Meta-analyses of randomized trials that compared telmisartan with other angiotensin receptor blockers report small improvements in fasting insulin, HOMA-IR and adiponectin (PMID: 29557807, PMID: 25151319, PMID: 21848583). A 3-week crossover trial in 12 men with impaired fasting glucose measured insulin action directly with a hyperinsulinemic euglycemic clamp and found no change with telmisartan (PMID: 20630944), and a 16-week placebo-controlled trial in 138 overweight adults found no difference in an insulin sensitivity index calculated from oral glucose tolerance testing, with clamp measurements in a subset of those participants (PMID: 20883237). In the TRANSCEND diabetes analysis of 3,488 participants free of diabetes, 21.8 percent on telmisartan and 22.4 percent on placebo developed diabetes over a median 56 months (PMID: 21788624).
What are the main side effects of telmisartan?
In placebo-controlled hypertension trials the adverse events more common with telmisartan than placebo were upper respiratory tract infection, back pain, sinusitis, diarrhea and pharyngitis, and fewer patients stopped telmisartan than placebo for adverse events (US prescribing information for telmisartan tablets). Telmisartan can cause symptomatic hypotension, particularly in people who are volume or salt depleted, and can raise serum potassium (US prescribing information for telmisartan tablets). Reports after approval include angioedema with fatal outcome, acute renal failure, hyperkalemia, hypoglycemia in diabetic patients and rare rhabdomyolysis (US prescribing information for telmisartan tablets). The telmisartan label carries a boxed warning for fetal toxicity.
Has telmisartan been tested for brain aging or dementia?
Telmisartan has been tested in small studies rather than dedicated dementia trials: a 20-patient randomized comparison against amlodipine over 6 months found stable cognitive scores on telmisartan and higher regional cerebral blood flow in several areas (PMID: 21929736). A claims analysis of 5.62 million people aged 60 or over linked moderate or high telmisartan exposure to fewer Alzheimer's disease diagnoses in African Americans but not in non-Hispanic European Americans, and the authors call for randomized trials (PMID: 36331056). Readers comparing telmisartan with other compounds studied in brain aging can also see BodyHackGuide's J147 guide, which covers a Salk Institute compound tested in a transgenic Alzheimer's disease mouse model (PMID: 22194796).
