Clomiphene citrate (clomifene) is a nonsteroidal selective estrogen receptor modulator (SERM) developed by the drug company Merrell and approved by the FDA as Clomid on 1 February 1967 to treat ovulatory dysfunction in women who want to become pregnant.

    01Overview

    What Clomiphene Citrate is

Clomiphene citrate is a mixture of two geometric isomers, zuclomiphene (cis, 30 to 50 percent of the drug) and enclomiphene (trans), and zuclomiphene was still detectable in plasma a month after a single dose (US prescribing information for clomiphene citrate tablets; PMID: 3091405). In women with polycystic ovary syndrome, clomiphene produced more live births than metformin in a 626-woman trial (22.5 against 7.2 percent) but fewer than letrozole in a 750-woman trial (19.1 against 27.5 percent) (PMID: 17287476, PMID: 25006718). In men, clomiphene is used off-label to raise testosterone, and a meta-analysis of randomized trials of clomiphene or enclomiphene found higher testosterone, LH and FSH than with placebo, while the US label states that no adequate or well-controlled studies show that clomiphene works for male infertility (PMID: 34933414, PMID: 41066380; US prescribing information for clomiphene citrate tablets).

    How it works

    Blocking estrogen feedback

Clomiphene interacts with estrogen receptors in the hypothalamus, pituitary, ovary, endometrium, vagina and cervix and may compete with estrogen for receptor binding; in women, the first endocrine response is a rise in pituitary gonadotropins, followed by follicle growth, a rise in estradiol and a gonadotropin surge that leads to ovulation (US prescribing information for clomiphene citrate tablets). The label states that both the estrogenic and the antiestrogenic properties of clomiphene may take part in starting ovulation (US prescribing information for clomiphene citrate tablets). Clomiphene began in the 1960s as one of several nonsteroidal antiestrogens studied by drug companies as possible postcoital contraceptives, after they prevented pregnancy in rats and mice, and in women it did the opposite and became the first medicine to induce ovulation in subfertile women (PMID: 35559738).

    In men

In men, clomiphene blocks estrogen receptors and reduces estrogen's negative feedback on the hypothalamus and pituitary, which raises gonadotropin secretion and the testes' own testosterone production (PMID: 42387872). In 12 healthy men aged 25 to 38 who took clomiphene daily for 30 days in an anti-doping study, mean testosterone rose 146 percent, LH 177 percent and FSH 170 percent above baseline (PMID: 30295816). Estradiol rises as well: in 15 men on long-term clomiphene, median estradiol went from 17.0 to 34.0 pg/mL as median testosterone went from 205 to 488 ng/dL (PMID: 27511863).

    Two isomers with different actions

The label states that clomiphene citrate contains 30 to 50 percent of the cis isomer, zuclomiphene, with the rest the trans isomer, enclomiphene, and that some data suggest zuclomiphene has greater estrogenic activity than enclomiphene (US prescribing information for clomiphene citrate tablets). In ovariectomized rats, zuclomiphene acted as a potent estrogen agonist on every tissue tested, while enclomiphene reduced the estrogenic effect of zuclomiphene on the uterus (PMID: 9724022). A 2019 review attributes most of clomiphene's benefit in men to enclomiphene and states that zuclomiphene contributes little to the intended outcomes (PMID: 31063005). BodyHackGuide covers the trans isomer separately in its enclomiphene guide.

    Metabolism and persistence

In human liver tissue, the enzyme CYP2D6 formed clomiphene's most potent antiestrogenic metabolites, (E)-4-hydroxyclomiphene and (E)-4-hydroxy-N-desethylclomiphene, and in a volunteer study, women with nonfunctional CYP2D6 genes had 8 to 12 times lower peak levels of these metabolites (PMID: 22108178). In liver microsome tests, the CYP2D6 inhibitor quinidine blocked the metabolism of enclomiphene completely (PMID: 18445989). After a single dose in 24 healthy women, zuclomiphene peaked later and was eliminated much more slowly than enclomiphene, with plasma levels still detected a month later (PMID: 3091405). In 9 women with polycystic ovary syndrome, a conventional half-life could not be calculated because of a very flat terminal phase, which the authors linked to the drug's fat solubility and wide distribution in the body (PMID: 19033451). In 15 men on long-term clomiphene, median serum zuclomiphene was 20 times the enclomiphene level (44.0 against 2.2 ng/mL); three of the nine authors listed Repros Therapeutics as their affiliation (PMID: 27511863).

    08Questions

    Frequently asked questions

    What is clomiphene citrate?

Clomiphene citrate (clomifene) is a selective estrogen receptor modulator approved by the FDA in 1967 as Clomid for ovulatory dysfunction in women who want to become pregnant (Drugs@FDA NDA 016131; US prescribing information for clomiphene citrate tablets). Clomiphene became the first medicine to induce ovulation in subfertile women (PMID: 35559738). In the US, clomiphene citrate is sold as generic prescription tablets, and the use of clomiphene to raise testosterone in men is off-label (Drugs@FDA; PMID: 34933414).

    What are enclomiphene and zuclomiphene?

Clomiphene citrate is a mixture of two geometric isomers, the trans isomer enclomiphene and the cis isomer zuclomiphene, which makes up 30 to 50 percent of clomiphene citrate (US prescribing information for clomiphene citrate tablets). In ovariectomized rats, zuclomiphene acted as a potent estrogen agonist, while enclomiphene opposed the estrogenic effect of zuclomiphene on the uterus (PMID: 9724022). Zuclomiphene is eliminated much more slowly than enclomiphene, and in 15 men on long-term clomiphene, serum zuclomiphene was 20 times the enclomiphene level (PMID: 3091405, PMID: 27511863). BodyHackGuide's enclomiphene guide covers the trans isomer of clomiphene on its own.

Does clomiphene raise testosterone in men?

In 12 healthy men, 30 days of clomiphene raised mean testosterone by 146 percent, LH by 177 percent and FSH by 170 percent (PMID: 30295816). In a placebo-controlled crossover trial in obese men with low testosterone, testosterone rose from 3.03 to 5.99 ng/mL on clomiphene and did not change on placebo (PMID: 28886024). A 2025 meta-analysis of randomized trials found that clomiphene or enclomiphene raised total testosterone by about 274 ng/dL more than placebo, with no difference from testosterone gel (PMID: 41066380). Clomiphene is not FDA approved for use in men, and its use in men is off-label (US prescribing information for clomiphene citrate tablets; PMID: 34933414).

    Does clomiphene help male infertility?

In a 1992 World Health Organization trial of 190 couples, pregnancy rates were 8.1 percent when the man took clomiphene and 11.7 percent with placebo, and semen quality did not differ (PMID: 1516979). A 2013 meta-analysis of 11 randomized trials of clomiphene or tamoxifen found a higher pregnancy rate with these estrogen antagonists (odds ratio 2.42) (PMID: 23970453). A 2021 systematic review found that semen measures fell in 17 to 24 percent of men treated with clomiphene, and that the decline persisted after stopping clomiphene in up to 17 percent (PMID: 33108678). The clomiphene label states that no adequate or well-controlled studies show effectiveness in male infertility (US prescribing information for clomiphene citrate tablets).

What side effects have been reported in men taking clomiphene?

In a review of 400 men treated for low testosterone, 8 percent of those on clomiphene for more than 3 years reported side effects, most often mood changes, blurred vision and breast tenderness (PMID: 31216250). Polycythemia occurred in 1.7 percent of 188 men on clomiphene and 11.2 percent of 175 men on testosterone replacement in a retrospective comparison (PMID: 27984109). Case reports describe venous blood clots and presumed optic nerve damage in men taking clomiphene (PMID: 27873748, PMID: 2875958, PMID: 34278201). The clomiphene label notes reports of gynecomastia and testicular tumors in men, and states that the cause and effect relationship between clomiphene and the testicular tumors is not known (US prescribing information for clomiphene citrate tablets).

How does clomiphene compare with letrozole for ovulation induction?

In a 750-woman trial in polycystic ovary syndrome, cumulative live births were 19.1 percent with clomiphene and 27.5 percent with letrozole, and hot flushes were more common with clomiphene (PMID: 25006718). In a 900-woman trial in unexplained infertility, live birth rates were 23.3 percent with clomiphene and 18.7 percent with letrozole, and clinical pregnancy rates with letrozole did not differ significantly from clomiphene (p=0.10) (PMID: 26398071). Twin pregnancies were 7.4 percent with clomiphene and 3.4 percent with letrozole in the polycystic ovary syndrome trial, a difference that was not significant (PMID: 25006718).

    Is clomiphene banned in sport?

WADA's 2026 Prohibited List names clomifene (clomiphene) as an anti-estrogenic substance prohibited at all times (WADA Prohibited List 2026, class S4.2). In an anti-doping study of 12 men who took clomiphene for 30 days, the urinary detection window for the clomiphene isomer zuclomiphene ranged from 121 to more than 261 days (PMID: 30295816).
