# Nicergoline Side Effects

Complete Nicergoline safety profile: reported side effects from published research, contraindications, severity tiers, and how to minimize adverse reactions through proper dosing and vendor selection.

## Reported Nicergoline side effects

In trials The 2001 Cochrane review found a mildly higher risk of adverse events on nicergoline than on placebo in 1,427 patients (odds ratio 1.51) (PMID: 11687175). A 2014 meta-analysis of 29 randomized trials, most from Europe and in cerebrovascular disease or dementia, found any adverse event slightly more often on nicergoline than on placebo (risk ratio 1.05) and serious adverse events less often (risk ratio 0.85), neither difference significant; anxiety was significantly less frequent on nicergoline, low blood pressure (against placebo) and hot flushes (against all comparators) were slightly more frequent without significance, and no included trial reported fibrosis or ergotism (PMID: 25079927). In a 12-month Italian trial, adverse events were recorded in 7 percent on nicergoline and 2 percent on placebo (PMID: 27519492). A 2014 review lists temporary rises in blood pressure, slow heart rate, low blood pressure and fainting among the cardiovascular events reported with nicergoline, mostly minor (PMID: 25243157). Fibrosis A French case series covering 1985 to 1995 described eight people who developed lung or pleural changes during long-term treatment at regular doses with ergot drugs, four of them on nicergoline; six had thickening of the pleura (the lining of the lungs) with or without fluid, all eight improved slowly after the drug was stopped, with slight residual pleural thickening in five of the six. The authors concluded that nicergoline and dihydroergotamine can cause chronic pleural thickening or effusion that slowly improves after withdrawal (PMID: 8793465). In its 2013 review the EMA counted 30 reports of fibrotic events with nicergoline, mostly in the lungs (pulmonary fibrosis 9, interstitial lung disease 6, pleural effusion 6, pleural fibrosis 5); in 21 of them a link to nicergoline could not be excluded, and these occurred after 5 months to 30 years of treatment in people aged 59 to 90, at the recommended daily dose where recorded (EMA assessment report EMA/750632/2013). Ten of the lung cases without another explanation improved 3 to 10 months after nicergoline was stopped, and one patient died after a diagnosis of pulmonary fibrosis suspected to be linked to nicergoline and amiodarone (EMA assessment report EMA/750632/2013). The EMA judged nicergoline to be associated with a risk of fibrotic reactions and noted probable under-reporting, because fibrosis develops slowly and is diagnosed late (EMA assessment report EMA/750632/2013). Ergotism No report in the manufacturer's safety database used the term ergotism with nicergoline, but 90 of 390 spontaneous reports collected over 40 years contained terms that can indicate ergotism, such as tingling, ischaemia, cold sensation, gangrene and vasospasm, and the EMA concluded that ergotism could not be ruled out (EMA assessment report EMA/750632/2013). Pregnancy PubMed searches on 27 October 2026 found no study of nicergoline in human pregnancy; the published pregnancy studies were rat studies of diabetic pregnancy (PMID: 2140805, PMID: 1969751).

## When to continue, reduce, or stop Nicergoline

Mild: continue protocol

Transient mild symptoms (slight GI upset, minor injection-site reaction, brief fatigue) typically resolve within 3-7 days as physiology adjusts. Continue at current dose; monitor weekly.

Moderate: reduce dose

Persistent mild symptoms past two weeks, or new symptoms like sustained mood changes, sleep disruption, or appetite shifts. Reduce dose by 25-50% and reassess at the end of the next 2-week window.

Severe: discontinue + seek care

Any signal of allergic response (rash, swelling, breathing difficulty), unexplained chest discomfort, severe injection-site reaction with infection signs, or vision changes. Discontinue immediately and consult a licensed practitioner.

## Nicergoline contraindications

There is no FDA label for nicergoline. The cautions that follow come from the EMA's 2013 review and from published studies. The EMA judged nicergoline to be associated with a risk of fibrotic reactions, mainly in the lungs and pleura, after months to years of treatment (EMA assessment report EMA/750632/2013). Among spontaneous reports with nicergoline, the EMA could not rule out ergotism (symptoms of ergot poisoning, such as spasms and obstructed blood circulation) (EMA assessment report EMA/750632/2013; EMA, ergot derivatives Article 31 referral). Nicergoline blocks alpha1-adrenergic receptors (PMID: 10027088), low blood pressure was slightly more frequent than on placebo in trials (PMID: 25079927), and low blood pressure, slow heart rate and fainting are among reported cardiovascular events (PMID: 25243157). In 10 healthy volunteers, nicergoline strengthened the effect of the beta blocker propranolol on cardiac output and heart oxygen demand (PMID: 6876944). Conversion of nicergoline's metabolite MMDL to MDL depends mainly on CYP2D6, and CYP2D6 poor metabolizers did not form measurable MDL (PMID: 8971425). No human pregnancy data were found on PubMed. A recent phase 3 dysphagia trial in Thailand excluded people with systolic blood pressure below 100 mmHg, heart rate below 50 a minute, active bleeding, anticoagulant use, chronic gout or high uric acid, creatinine clearance below 30 ml/min, moderate or severe liver impairment, and pregnancy or breastfeeding (ClinicalTrials.gov NCT05551182).

## Why vendor quality drives Nicergoline side-effect frequency

Research-compound communities consistently report that adverse-event frequency tracks with source quality. Three reasons: (1) impurity profiles vary by lab (residual synthesis byproducts can trigger reactions); (2) mislabeled mass means actual dose can be 50-150% of stated dose; (3) reconstitution-grade water and capping methodology affect long-term storage stability. COA-verified vendors with published HPLC data minimize all three failure modes. Check a vendor's COA record and Trust Score before you buy.

## Nicergoline side effects FAQ

### What are the most common Nicergoline side effects?

In trials The 2001 Cochrane review found a mildly higher risk of adverse events on nicergoline than on placebo in 1,427 patients (odds ratio 1.51) (PMID: 11687175). A 2014 meta-analysis of 29 randomized trials, most from Europe and in cerebrovascular disease or dementia, found any adverse event slightly more often on nicergoline than on placebo (risk ratio 1.05) and serious adverse events less oft

### Are there severe side effects to watch for with Nicergoline?

Severe reactions are rare in published research but can occur. Watch for: allergic-type symptoms (rash, swelling, breathing difficulty), unexplained chest discomfort, infection signs at injection sites, vision changes, or severe persistent mood/sleep disruption. Any of these warrants immediate discontinuation and consultation with a licensed practitioner. The Severity Tiers section above triages mild → moderate → severe response patterns.

### Does Nicergoline have contraindications?

There is no FDA label for nicergoline. The cautions that follow come from the EMA's 2013 review and from published studies. The EMA judged nicergoline to be associated with a risk of fibrotic reactions, mainly in the lungs and pleura, after months to years of treatment (EMA assessment report EMA/750632/2013). Among spontaneous reports with nicergoline, the EMA could not rule out ergotism (symptoms

### How can I minimize Nicergoline side effects?

Three biggest levers: (1) Start at the Beginner-tier dose for 2-3 weeks before escalating, which gives time to detect adverse signals at minimum effective dose. (2) Source from COA-verified vendors only: impurity profiles and mislabeled compounds drive a disproportionate share of reported side effects in research-compound communities. (3) Maintain adequate cycle off-time when applicable: receptor-saturating compounds benefit from 4-6 week breaks. The vendor section below lists the cheapest in-stock Nicergoline sources we track, each linked to its Trust Score (COA verification is 30% of the score).
