2026 Safety Guide

# L-Citrulline Side Effects

Complete L-Citrulline safety profile: reported side effects from published research, contraindications, severity tiers, and how to minimize adverse reactions through proper dosing and vendor selection.

## Reported L-Citrulline side effects

In human trials In 8 healthy men, single doses of 2 to 15 g caused no side effects (PMID: 17953788). In a study in Japan, healthy men took graded amounts of 6, 12, 18 and 24 g a day for 4 weeks each; citrulline caused only minor laboratory changes (in lactate dehydrogenase and plasma amino acids) and no treatment-related adverse events, and the authors set a no-observed-adverse-effect level at the highest amount tested, 24 g a day for 4 weeks (PMID: 36571619). Four of that study's five authors were employees of amino acid manufacturers (PMID: 36571619). In a crossover trial of 41 men, 6 (14.6 percent) reported stomach discomfort after a single 8 g dose of citrulline malate (PMID: 20386132), while none of 10 trained men had gastrointestinal side effects after 12 g (PMID: 26808848). A 2026 systematic review of 12 randomized trials in 360 postmenopausal women found no adverse effects attributed to citrulline (PMID: 41588439). A 2007 review of gastrointestinal effects noted that oral arginine and citrulline both raise nitric oxide production in the small intestine, and that most reported side effects of arginine followed single doses above 9 g (PMID: 17513449). Intravenous use in children None of 17 infants and children given intravenous citrulline during heart surgery had the prespecified safety event, a 20 percent drop in mean arterial blood pressure (PMID: 17662768). In the 189-child phase 3 trial, serious adverse events occurred in 11 of 99 children on citrulline and 6 of 90 on placebo (ClinicalTrials.gov NCT02891837 results). Long-term clinical use In a Paris reference center's review of 79 patients with urea cycle disorders treated with citrulline, arginine or both over a median follow-up of 9.5 years, 2 patients had treatment-related gastrointestinal reactions (PMID: 37480106). Product quality A 2016 review led by United States Pharmacopeia scientists used L-citrulline as one of two amino acids linked to adverse events on the market to argue for quality specifications (PMID: 27934657). The authors of a 2018 citrulline malate trial reported that chemical analyses of the products they reviewed differed from the manufacturers' claims (PMID: 30097067).

## When to continue, reduce, or stop L-Citrulline

Mild: continue protocol

Transient mild symptoms (slight GI upset, minor injection-site reaction, brief fatigue) typically resolve within 3-7 days as physiology adjusts. Continue at current dose; monitor weekly.

Moderate: reduce dose

Persistent mild symptoms past two weeks, or new symptoms like sustained mood changes, sleep disruption, or appetite shifts. Reduce dose by 25-50% and reassess at the end of the next 2-week window.

Severe: discontinue + seek care

Any signal of allergic response (rash, swelling, breathing difficulty), unexplained chest discomfort, severe injection-site reaction with infection signs, or vision changes. Discontinue immediately and consult a licensed practitioner.

## L-Citrulline contraindications

No drug label exists for L-citrulline, so no formal contraindications have been set. Cautions in the literature: Blood pressure medicines and low blood pressure: L-citrulline lowered systolic blood pressure by about 4 mmHg in a pooled analysis of small trials (PMID: 30788274), and a 2018 review called for studies of L-citrulline taken with drugs for hypertension and cardiovascular disease (PMID: 30029482). PDE5 inhibitors: L-citrulline raised urinary cGMP in healthy volunteers (PMID: 17662090), the messenger whose breakdown by PDE5 these drugs block (PMID: 25043200); PubMed lists no study of the combination's effect on blood pressure. Pregnancy: a 2025 meta-analysis found only one L-citrulline trial in pregnancy, in 36 women, which reported no effect on pre-eclampsia or blood pressure (PMID: 39800868). Long-term use: most citrulline trials lasted weeks, and a 2018 review stated that the safety and efficacy of long-term supplementation need further study (PMID: 30029482). Inherited urea cycle disorders: citrulline is part of specialist treatment for some of these disorders, and the choice between citrulline and arginine depends on the missing enzyme (PMID: 16082501; PMID: 37480106).

## Why vendor quality drives L-Citrulline side-effect frequency

Research-compound communities consistently report that adverse-event frequency tracks with source quality. Three reasons: (1) impurity profiles vary by lab (residual synthesis byproducts can trigger reactions); (2) mislabeled mass means actual dose can be 50-150% of stated dose; (3) reconstitution-grade water and capping methodology affect long-term storage stability. COA-verified vendors with published HPLC data minimize all three failure modes. Check a vendor's COA record and Trust Score before you buy.

## L-Citrulline side effects FAQ

### What are the most common L-Citrulline side effects?

In human trials In 8 healthy men, single doses of 2 to 15 g caused no side effects (PMID: 17953788). In a study in Japan, healthy men took graded amounts of 6, 12, 18 and 24 g a day for 4 weeks each; citrulline caused only minor laboratory changes (in lactate dehydrogenase and plasma amino acids) and no treatment-related adverse events, and the authors set a no-observed-adverse-effect level at the

### Are there severe side effects to watch for with L-Citrulline?

Severe reactions are rare in published research but can occur. Watch for: allergic-type symptoms (rash, swelling, breathing difficulty), unexplained chest discomfort, infection signs at injection sites, vision changes, or severe persistent mood/sleep disruption. Any of these warrants immediate discontinuation and consultation with a licensed practitioner. The Severity Tiers section above triages mild → moderate → severe response patterns.

### Does L-Citrulline have contraindications?

No drug label exists for L-citrulline, so no formal contraindications have been set. Cautions in the literature: Blood pressure medicines and low blood pressure: L-citrulline lowered systolic blood pressure by about 4 mmHg in a pooled analysis of small trials (PMID: 30788274), and a 2018 review called for studies of L-citrulline taken with drugs for hypertension and cardiovascular disease (PMID: 3

### How can I minimize L-Citrulline side effects?

Three biggest levers: (1) Start at the Beginner-tier dose for 2-3 weeks before escalating, which gives time to detect adverse signals at minimum effective dose. (2) Source from COA-verified vendors only: impurity profiles and mislabeled compounds drive a disproportionate share of reported side effects in research-compound communities. (3) Maintain adequate cycle off-time when applicable: receptor-saturating compounds benefit from 4-6 week breaks. The vendor section below lists the cheapest in-stock L-Citrulline sources we track, each linked to its Trust Score (COA verification is 30% of the score).
