# Ketoconazole Side Effects

Complete Ketoconazole safety profile: reported side effects from published research, contraindications, severity tiers, and how to minimize adverse reactions through proper dosing and vendor selection.

## Reported Ketoconazole side effects

Shampoo and other topical forms In 11 double-blind trials in 264 patients using 2 percent shampoo for dandruff or seborrheic dermatitis, an increase in normal hair loss and irritation occurred in fewer than 1 percent of patients; in three open-label safety trials in which 41 patients shampooed 4 to 10 times a week for 6 months, abnormal hair texture, scalp pustules, mild skin dryness and itching each occurred once (US prescribing information for ketoconazole 2% shampoo). Reports after marketing include hair discoloration, abnormal hair texture and loss of curl in permanently waved hair; itching and burning; contact dermatitis, angioedema, hair loss (alopecia), rash, hives and dry skin; and severe hypersensitivity reactions including anaphylaxis (US prescribing information for ketoconazole 2% shampoo). In a 246-patient dandruff trial, all 9 adverse experiences occurred with selenium sulfide 2.5 percent shampoo and none with ketoconazole 2 percent shampoo (PMID: 8245236). A 2019 systematic review of 40 randomized trials of topical ketoconazole with 4,566 patients judged it safe but noted that allergic contact dermatitis can occur (PMID: 30668185). Oral tablets: liver injury The tablet label's boxed warning states that serious liver injury (including deaths and liver transplants) has occurred with oral ketoconazole, sometimes in patients without obvious risk factors, and its warnings section adds that this happened both with high doses over short periods and with low doses over long periods (US prescribing information for ketoconazole tablets). In an analysis of 54 reports of liver injury submitted to the FDA, 33 of them judged likely to be caused by ketoconazole, the authors put symptomatic, potentially serious liver injury at perhaps 1 in 15,000 exposed people and reported a published estimate of 5 to 10 percent for mild, reversible liver enzyme rises (PMID: 6319220). In a UK cohort of 69,830 users of oral antifungals, acute liver injury occurred at 134.1 per 100,000 person-months of ketoconazole use, based on 2 cases, a relative risk of 228 against nonusers (PMID: 10594489). A 2013 meta-analysis of 204 studies put the incidence of ketoconazole-related liver toxicity at 3.6 to 4.2 percent (PMID: 23895707). The EMA's 2013 review found that liver injury with oral ketoconazole was more frequent and more serious than with other antifungals and could start soon after treatment began at recommended doses (EMA referral on ketoconazole-containing medicines). Oral tablets: hormones and adrenal glands Pont and colleagues proposed that breast enlargement in men taking ketoconazole comes from a rise in the ratio of estradiol to testosterone, after 600 mg doses lowered testosterone much more than estradiol in four volunteers (PMID: 4040740). The tablet label lists breast enlargement and menstrual disorders among trial reactions and adrenocortical insufficiency, erectile dysfunction and, above recommended doses, absent sperm (azoospermia) among reports after approval (US prescribing information for ketoconazole tablets). In a trial of 350 men with metastatic prostate cancer given 1,200 mg a day, 11 died within two weeks of starting, and the label states that it could not be determined whether these deaths were caused by ketoconazole or adrenal insufficiency (US prescribing information for ketoconazole tablets). Oral tablets: heart rhythm and other effects At 200 mg twice a day for 3 to 7 days, ketoconazole raised the QTc interval by a mean maximum of about 6 to 12 milliseconds, and combining it with certain drugs can cause life-threatening rhythm problems such as torsades de pointes (US prescribing information for ketoconazole tablets). Other reported reactions include anaphylaxis after the first dose, nausea and vomiting, abdominal pain, headache, acute generalized exanthematous pustulosis and photosensitivity (US prescribing information for ketoconazole tablets).

## When to continue, reduce, or stop Ketoconazole

Mild: continue protocol

Transient mild symptoms (slight GI upset, minor injection-site reaction, brief fatigue) typically resolve within 3-7 days as physiology adjusts. Continue at current dose; monitor weekly.

Moderate: reduce dose

Persistent mild symptoms past two weeks, or new symptoms like sustained mood changes, sleep disruption, or appetite shifts. Reduce dose by 25-50% and reassess at the end of the next 2-week window.

Severe: discontinue + seek care

Any signal of allergic response (rash, swelling, breathing difficulty), unexplained chest discomfort, severe injection-site reaction with infection signs, or vision changes. Discontinue immediately and consult a licensed practitioner.

## Ketoconazole contraindications

The tablet label contraindicates oral ketoconazole in acute or chronic liver disease, and it advises against alcohol and, where possible, other drugs that can harm the liver during treatment (US prescribing information for ketoconazole tablets). Combining the tablets with any of these drugs is contraindicated on the label: dofetilide, quinidine, pimozide, lurasidone, cisapride, methadone, disopyramide, dronedarone, ranolazine, ergot alkaloids, irinotecan, oral midazolam, alprazolam, triazolam, felodipine, nisoldipine, tolvaptan, eplerenone, lovastatin, simvastatin and colchicine (US prescribing information for ketoconazole tablets). The tablet label warns that ketoconazole lowers adrenal corticosteroid output at doses of 400 mg or more and calls for adrenal monitoring in people with adrenal insufficiency, borderline adrenal function or prolonged stress such as major surgery (US prescribing information for ketoconazole tablets). The tablet label reports liver toxicity (including deaths and liver transplants) in patients given oral ketoconazole for unapproved uses: nail fungus, skin dermatophyte infections or Candida infections, which the tablets are not indicated to treat (US prescribing information for ketoconazole tablets). As for pregnancy, oral ketoconazole caused limb malformations (syndactylia and oligodactylia) in rats at 80 mg/kg a day, possibly related to maternal toxicity, while ketoconazole was not detected in plasma after long-term shampooing (US prescribing information for ketoconazole 2% shampoo). The 2 percent shampoo is contraindicated in people with known hypersensitivity to its ingredients, can irritate the eyes, and has been reported to remove the curl from permanently waved hair (US prescribing information for ketoconazole 2% shampoo).

## Why vendor quality drives Ketoconazole side-effect frequency

Research-compound communities consistently report that adverse-event frequency tracks with source quality. Three reasons: (1) impurity profiles vary by lab (residual synthesis byproducts can trigger reactions); (2) mislabeled mass means actual dose can be 50-150% of stated dose; (3) reconstitution-grade water and capping methodology affect long-term storage stability. COA-verified vendors with published HPLC data minimize all three failure modes. Check a vendor's COA record and Trust Score before you buy.

## Ketoconazole side effects FAQ

### What are the most common Ketoconazole side effects?

Shampoo and other topical forms In 11 double-blind trials in 264 patients using 2 percent shampoo for dandruff or seborrheic dermatitis, an increase in normal hair loss and irritation occurred in fewer than 1 percent of patients; in three open-label safety trials in which 41 patients shampooed 4 to 10 times a week for 6 months, abnormal hair texture, scalp pustules, mild skin dryness and itching e

### Are there severe side effects to watch for with Ketoconazole?

Severe reactions are rare in published research but can occur. Watch for: allergic-type symptoms (rash, swelling, breathing difficulty), unexplained chest discomfort, infection signs at injection sites, vision changes, or severe persistent mood/sleep disruption. Any of these warrants immediate discontinuation and consultation with a licensed practitioner. The Severity Tiers section above triages mild → moderate → severe response patterns.

### Does Ketoconazole have contraindications?

The tablet label contraindicates oral ketoconazole in acute or chronic liver disease, and it advises against alcohol and, where possible, other drugs that can harm the liver during treatment (US prescribing information for ketoconazole tablets). Combining the tablets with any of these drugs is contraindicated on the label: dofetilide, quinidine, pimozide, lurasidone, cisapride, methadone, disopyra

### How can I minimize Ketoconazole side effects?

Three biggest levers: (1) Start at the Beginner-tier dose for 2-3 weeks before escalating, which gives time to detect adverse signals at minimum effective dose. (2) Source from COA-verified vendors only: impurity profiles and mislabeled compounds drive a disproportionate share of reported side effects in research-compound communities. (3) Maintain adequate cycle off-time when applicable: receptor-saturating compounds benefit from 4-6 week breaks. The vendor section below lists the cheapest in-stock Ketoconazole sources we track, each linked to its Trust Score (COA verification is 30% of the score).
