2026 Safety Guide

# Bromelain Side Effects

Complete Bromelain safety profile: reported side effects from published research, contraindications, severity tiers, and how to minimize adverse reactions through proper dosing and vendor selection.

## Reported Bromelain side effects

Topical NexoBrid In the two adult trials behind the FDA approval (177 patients on NexoBrid, 149 on standard care), the adverse reactions more common with NexoBrid included itching (15 versus 13 percent), fever (12 versus 9 percent), wound complications such as graft loss (9 versus 6 percent), anemia (6 versus 5 percent) and vomiting (5 versus 3 percent) (NexoBrid FDA label). In the pediatric trial (69 children on NexoBrid, 70 on standard care), itching (13 versus 10 percent), fever (10 versus 6 percent) and vomiting (7 versus 4 percent) were more common with NexoBrid (NexoBrid FDA label). Serious allergic reactions, including anaphylaxis, have been reported with NexoBrid's use after approval (NexoBrid FDA label). The label asks health workers preparing the gel to avoid exposure, using gloves and masks as needed, and to rinse off any gel that touches the skin to lower the chance of skin sensitization (NexoBrid FDA label). A 2026 review lists procedural pain and allergic reactions among the product's main limitations (PMID: 42704059). Oral bromelain In a 12-week knee osteoarthritis trial, adverse events were generally mild (PMID: 17121765). In a German study of 116 children with sinusitis, one 10-year-old with a known pineapple allergy had a mild, self-limiting allergic reaction (PMID: 15796206). Across 7 randomized trials of bromelain alone or combined with other oral compounds in a 2023 systematic review, 11 participants reported side effects, mainly gastrointestinal, and 2 stopped treatment (PMID: 37202035). In a crossover trial after wisdom tooth removal, adverse events were not more frequent with bromelain than with placebo (PMID: 27601419). Allergy from work exposure Bromelain powder is an occupational allergen. A pharmaceutical worker who handled bromelain developed asthma and rhinitis and reacted both to an inhaled challenge with 0.03 mg of bromelain and to eating 190 g of pineapple, and 5 of 6 workers sensitized to papain also reacted to bromelain, which showed cross-reaction between the two plant proteases (PMID: 498486). Four workers in a blood grouping laboratory developed occupational allergy to bromelain, and the authors concluded that bromelain is a strong sensitizer, that sensitization usually occurs by inhalation rather than ingestion, and that skin testing with it can cause whole-body reactions (PMID: 3233722). In a pharmaceutical factory, 7 of 10 workers sensitized to enzymes had positive skin prick tests to bromelain (PMID: 9298178). The sugar chain attached to bromelain (MUXF) is used in allergy testing to detect antibodies against cross-reactive plant carbohydrates (PMID: 21557753).

## When to continue, reduce, or stop Bromelain

Mild: continue protocol

Transient mild symptoms (slight GI upset, minor injection-site reaction, brief fatigue) typically resolve within 3-7 days as physiology adjusts. Continue at current dose; monitor weekly.

Moderate: reduce dose

Persistent mild symptoms past two weeks, or new symptoms like sustained mood changes, sleep disruption, or appetite shifts. Reduce dose by 25-50% and reassess at the end of the next 2-week window.

Severe: discontinue + seek care

Any signal of allergic response (rash, swelling, breathing difficulty), unexplained chest discomfort, severe injection-site reaction with infection signs, or vision changes. Discontinue immediately and consult a licensed practitioner.

## Bromelain contraindications

Contraindications on the NexoBrid label (topical prescription product): Known allergy to anacaulase-bcdb, bromelain or pineapple, or to any other component (NexoBrid FDA label). Known allergy to papaya or papain, because of the risk of cross-sensitivity (NexoBrid FDA label). Cautions in the literature: Bleeding and clotting: the NexoBrid label advises avoiding the product in uncontrolled clotting disorders and using caution with anticoagulants, other drugs that affect clotting, low platelet counts or other bleeding risks, citing reports of reduced platelet aggregation and fibrinogen after oral bromelain (NexoBrid FDA label); laboratory studies found reduced platelet aggregation in human blood (PMID: 16308185; PMID: 10218125). Pineapple and papain allergy: a child with known pineapple allergy had a mild allergic reaction to bromelain given for sinusitis (PMID: 15796206), and bromelain and papain cross-reacted in sensitized workers (PMID: 498486). Inhaled powder: bromelain dust caused occupational asthma in laboratory and pharmaceutical workers (PMID: 3233722; PMID: 498486). Pregnancy and breastfeeding: the NexoBrid label reports no human data on use in pregnancy and no data on presence in milk (NexoBrid FDA label).

## Why vendor quality drives Bromelain side-effect frequency

Research-compound communities consistently report that adverse-event frequency tracks with source quality. Three reasons: (1) impurity profiles vary by lab (residual synthesis byproducts can trigger reactions); (2) mislabeled mass means actual dose can be 50-150% of stated dose; (3) reconstitution-grade water and capping methodology affect long-term storage stability. COA-verified vendors with published HPLC data minimize all three failure modes. Check a vendor's COA record and Trust Score before you buy.

## Bromelain side effects FAQ

### What are the most common Bromelain side effects?

Topical NexoBrid In the two adult trials behind the FDA approval (177 patients on NexoBrid, 149 on standard care), the adverse reactions more common with NexoBrid included itching (15 versus 13 percent), fever (12 versus 9 percent), wound complications such as graft loss (9 versus 6 percent), anemia (6 versus 5 percent) and vomiting (5 versus 3 percent) (NexoBrid FDA label). In the pediatric trial

### Are there severe side effects to watch for with Bromelain?

Severe reactions are rare in published research but can occur. Watch for: allergic-type symptoms (rash, swelling, breathing difficulty), unexplained chest discomfort, infection signs at injection sites, vision changes, or severe persistent mood/sleep disruption. Any of these warrants immediate discontinuation and consultation with a licensed practitioner. The Severity Tiers section above triages mild → moderate → severe response patterns.

### Does Bromelain have contraindications?

Contraindications on the NexoBrid label (topical prescription product): Known allergy to anacaulase-bcdb, bromelain or pineapple, or to any other component (NexoBrid FDA label). Known allergy to papaya or papain, because of the risk of cross-sensitivity (NexoBrid FDA label). Cautions in the literature: Bleeding and clotting: the NexoBrid label advises avoiding the product in uncontrolled clotting

### How can I minimize Bromelain side effects?

Three biggest levers: (1) Start at the Beginner-tier dose for 2-3 weeks before escalating, which gives time to detect adverse signals at minimum effective dose. (2) Source from COA-verified vendors only: impurity profiles and mislabeled compounds drive a disproportionate share of reported side effects in research-compound communities. (3) Maintain adequate cycle off-time when applicable: receptor-saturating compounds benefit from 4-6 week breaks. The vendor section below lists the cheapest in-stock Bromelain sources we track, each linked to its Trust Score (COA verification is 30% of the score).
