01 Overview

## What Agmatine Sulfate is

In laboratory studies of animal tissue and cells, agmatine binds alpha-2 adrenergic and imidazoline receptors, blocks NMDA glutamate receptor channels and inhibits inducible nitric oxide synthase (PMID: 7906055, PMID: 10455281, PMID: 8699639). Agmatine sulfate is sold in the United States as a dietary supplement ingredient and is not an FDA-approved drug (PMID: 39286406; FDA structure/function notification dockets and Drugs@FDA, searched 25 October 2026). Most agmatine research has been done in animals and cells (PMID: 28786346), and the published human studies of agmatine sulfate alone include one placebo-controlled sciatica trial that analyzed 61 people and small uncontrolled reports, most involving one research group (PMID: 20447305, PMID: 32102167, PMID: 25247837).

### How it works

### What agmatine is

Agmatine is decarboxylated arginine: the enzyme arginine decarboxylase removes a carboxyl group from the amino acid arginine, and the enzyme agmatinase breaks agmatine down to putrescine (PMID: 17025265). The molecule was first described in 1910 (PMID: 30915570). Agmatine sulfate, the form sold as a supplement and used in most human studies, is agmatine combined one to one with sulfuric acid (PubChem CID 2794990).

Read the full mechanism Show less

Arginine decarboxylase activity is low in mammals, and researchers have suggested that much of the body's agmatine comes from food and gut bacteria (PMID: 30915570). Bacteria and plants make agmatine by decarboxylating arginine (PMID: 30923709), and agmatine is present in low amounts in many foods (PMID: 39286406). A 2017 review described the mammalian enzymes that make and break down agmatine as incompletely characterized (PMID: 28747403).

### Rediscovery as a brain signaling molecule

In 1994, Donald Reis's laboratory at Cornell purified a "clonidine-displacing substance" from cow brain and identified it as agmatine. Agmatine bound alpha-2 adrenergic and imidazoline receptors, the targets of the blood pressure drug clonidine, and the enzyme that makes it was found in brain (PMID: 7906055). A 2000 review from the same group summarized laboratory evidence that agmatine is made in mammalian brain, stored in synaptic vesicles, released on depolarization and inactivated by agmatinase, and proposed it as a neurotransmitter (PMID: 10785653).

### Targets in animal and cell studies

- **Alpha-2 and imidazoline receptors.** Agmatine binds both receptor types (PMID: 7906055, PMID: 10785653).
- **NMDA glutamate receptors.** In rat brain cells and in human cells engineered to carry NMDA receptors, agmatine blocked the receptor channel from a site inside its pore and protected neurons from glutamate toxicity in culture (PMID: 10455281).
- **Nitric oxide synthase.** In rat aorta and in enzyme partially purified from macrophages, agmatine inhibited the inducible form of nitric oxide synthase but not the constitutive form (PMID: 8699639).

### Getting into the brain

Agmatine is a polar molecule, and how it crosses the blood-brain barrier is not well understood (PMID: 15028573). A 2006 review described agmatine as rapidly broken down outside the brain and poor at entering it (PMID: 17025265). Later animal studies found agmatine in the brain after oral dosing: in mice, oral agmatine sulfate raised agmatine in the hippocampus and other brain regions (PMID: 31481723), and a 2023 rat study from the University of Minnesota concluded that agmatine adequately crosses biological barriers, with oral bioavailability of 29 to 35 percent (PMID: 37770201).

### Agmatine levels in human disease (observational)

These studies measured agmatine already in the blood; nobody was given agmatine. Plasma agmatine was higher in depressed patients than in healthy controls and returned toward normal with the antidepressant bupropion (PMID: 10415948). Plasma agmatine was higher in 18 people with schizophrenia than in 19 controls (PMID: 23664672), but a later report found lower blood agmatine in early-onset schizophrenia (PMID: 32446703). Plasma agmatine was higher during mania in 30 people with bipolar disorder than in 30 controls (PMID: 31081413), and lower in 34 autistic children than in 28 controls (PMID: 29302750). These are associations, not evidence of cause.

03 Safety

## Agmatine Sulfate side effects and interactions

### Side effects

#### Common

- Mild-to-moderate diarrhea and mild nausea in three people in the highest-dose group of a dose-escalation study (PMID: 20447305)
- Stomach discomfort and loose stools in one patient in a small depression pilot, resolving within days (PMID: 25287313)

#### Serious events

- No treatment-related adverse events in the placebo-controlled sciatica phase, and no serious events in a combination-product trial (PMID: 20447305, PMID: 42713349)
- Slight falls in blood pressure and body weight in rats after 95 days, reversing after treatment stopped (PMID: 24140462)

### Who should not use it

Contraindications

**Blood pressure and blood pressure medicines.** In animal tissue, agmatine binds the same alpha-2 adrenergic and imidazoline receptors as the blood pressure drug clonidine (PMID: 7906055). A 2025 review of 60 animal studies found that agmatine can raise or lower blood pressure and heart rate depending on dose and route (PMID: 41283257), and in rats, 95 days of agmatine sulfate lowered blood pressure slightly (PMID: 24140462). The 2010 human trial excluded people treated with medicines for hypertension (ClinicalTrials.gov NCT00405041).

Read all the contraindications and cautions Show less

**Opioid pain medicines.** In mice, agmatine made morphine more than five times as potent at relieving pain and prevented tolerance to it (PMID: 8720472).

**Seizure medicines.** In mice, agmatine strengthened the anticonvulsant effect of phenobarbital and valproate without changing their brain levels (PMID: 18379717).

**Psychiatric illness.** Blood agmatine was higher in small groups of people with schizophrenia and with mania than in controls, and the authors of those studies proposed that excess agmatine could contribute to these conditions (PMID: 23664672, PMID: 31081413). No human study of agmatine supplements in these conditions was found.

**Pregnancy and breastfeeding.** No human safety data were found; human studies excluded pregnant and breastfeeding women (ClinicalTrials.gov NCT00405041, NCT01524666).

08 Questions

## Frequently asked questions

What is agmatine sulfate?

Agmatine sulfate is the sulfate salt of agmatine, which the body and gut bacteria make from the amino acid arginine and which is also present in many foods (PMID: 17025265, PMID: 7906055, PMID: 30915570, PMID: 39286406). Agmatine was identified in cow brain in 1994 as a substance that, in animal tissue, binds the same alpha-2 adrenergic and imidazoline receptors as the drug clonidine (PMID: 7906055).

Is agmatine a supplement or a drug?

Agmatine sulfate is sold in the United States as a dietary supplement ingredient (PMID: 39286406; FDA structure/function claim notification dockets on regulations.gov). Agmatine is not an FDA-approved drug: a Drugs@FDA search on 25 October 2026 found no application for agmatine.

What human studies of agmatine exist?

The main human agmatine study is a 2010 sciatica trial in which 61 of 99 randomized participants were analyzed, and 14 days of agmatine sulfate were followed by greater improvement in pain and quality-of-life scores than placebo (PMID: 20447305). Other human agmatine data come from an 11-patient open-label neuropathy series, a depression pilot that reported three remitted patients, and a self-report by two researchers (PMID: 32102167, PMID: 25287313, PMID: 25247837). Most of these agmatine studies involved the same research group (PMID: 20447305, PMID: 32102167, PMID: 25247837).

Does agmatine help nerve pain?

In the 2010 trial in sciatica from a herniated disc, average pain measures improved by 26.7 percent with agmatine sulfate against 6.0 percent with placebo, but only 61 of 99 randomized participants were analyzed (PMID: 20447305). In an open-label series of 11 people with painful small fiber neuropathy, agmatine sulfate was followed by a 46.4 percent average reduction in pain scores, with no control group (PMID: 32102167). In rodents, agmatine reduced pain hypersensitivity after inflammation and nerve injury (PMID: 10984543).

What do animal studies of agmatine show?

In mice, agmatine strengthened morphine pain relief and prevented morphine tolerance (PMID: 8720472). In mice, agmatine also reduced immobility in tests used to screen antidepressants (PMID: 11930146). In rats, agmatine reduced brain damage after a temporary blockage of a brain artery (PMID: 20815926).

What side effects has agmatine shown?

In a 2010 dose-escalation study, three people in the highest agmatine sulfate dose group had mild-to-moderate diarrhea and mild nausea that stopped after treatment ended (PMID: 20447305). No treatment-related adverse events were noted in the placebo-controlled phase of that agmatine trial (PMID: 20447305). In rats given agmatine sulfate for 95 days, body weight and blood pressure fell slightly and recovered after treatment stopped (PMID: 24140462).

Does agmatine reach the brain?

A 2006 review described agmatine as poor at entering the brain (PMID: 17025265), but in mice oral agmatine sulfate raised agmatine levels in several brain regions (PMID: 31481723). A 2023 rat study measured oral agmatine bioavailability at 29 to 35 percent and concluded that agmatine adequately crosses biological barriers (PMID: 37770201). No study of agmatine levels in the human brain after supplements was found.

"text": "The main human agmatine study is a 2010 sciatica trial in which 61 of 99 randomized participants were analyzed, and 14 days of agmatine sulfate were followed by greater improvement in pain and quality-of-life scores than placebo (PMID: 20447305). Other human agmatine data come from an 11-patient open-label neuropathy series, a depression pilot that reported three remitted patients, and a self-report by two researchers (PMID: 32102167, PMID: 25287313, PMID: 25247837). Most of these agmatine studies involved the same research group (PMID: 20447305, PMID: 32102167, PMID: 25247837)."

In an open-label series of 11 people with painful small fiber neuropathy, agmatine sulfate was followed by a 46.4 percent average reduction in pain scores, with no control group (PMID: 32102167). In rodents, agmatine reduced pain hypersensitivity after inflammation and nerve injury (PMID: 10984543)."
