# Eloralintide: Amylin and the Phase 2 Weight-Loss Results

Eloralintide is an amylin agonist, not a GLP-1 drug. Examine its 263-person, 48-week phase 2 trial, the 20.1% headline and evidence gaps.

Key Takeaway

**Eloralintide is an investigational selective amylin receptor agonist, not a GLP-1 agonist.** Its phase 2 trial reported 9.5% to 20.1% mean weight loss at 48 weeks across treatment arms, versus 0.4% with placebo, under an efficacy estimand. The study did not demonstrate superiority to GLP-1 medicines.

Evidence checked October 21, 2026. “Elorinatide” is a misspelling sometimes used for eloralintide. We use the trial's actual name here. This is research reporting, not a treatment or dosing guide.

## What makes eloralintide different?

Eloralintide, also called LY3841136, targets amylin receptors. UBT251 and retatrutide instead target GLP-1, GIP and glucagon receptors. That distinction is useful when following trial programs; it does not tell us which medicine would work best for a particular person.

We link mechanisms to the evidence rather than assuming a new pathway means fewer adverse effects. The published Lancet phase 2 trial (https://pubmed.ncbi.nlm.nih.gov/41207310/) is the primary source for this article.

## What did the phase 2 trial measure?

The randomized, double-blind, placebo-controlled trial enrolled **263 adults at 46 US centers**. Eligibility included ages 18–75 and obesity, or overweight with at least one weight-related comorbidity. Participants did not have type 2 diabetes. Its primary endpoint was percentage body-weight change at week 48.

Our table keeps the headline attached to that population and analysis.

| Question | Evidence from this trial | Limit |
| --- | --- | --- |
| How many participants? | 263 adults | Too small to settle rare adverse effects |
| How long? | 48 weeks | Does not establish long-term maintenance after stopping |
| Mean loss across eloralintide arms? | 9.5%–20.1% under the efficacy estimand | The highest arm is not the whole trial's average |
| Placebo mean loss? | 0.4% | A within-trial comparator |
| Active comparator? | None | No direct ranking against semaglutide, tirzepatide or retatrutide |

Sources: Lancet paper (https://doi.org/10.1016/S0140-6736(25)02155-5) and Lilly's November 6, 2025 results announcement (https://investor.lilly.com/news-releases/news-release-details/lillys-selective-amylin-agonist-eloralintide-demonstrated).

## What does efficacy estimand mean?

An estimand defines the treatment effect researchers are trying to estimate, including how they handle events such as stopping treatment or starting other therapies. An efficacy estimand asks about an effect under specified adherence assumptions. It is not simply the observed average for every person who started, regardless of what happened next.

For that reason, we label 20.1% with its analysis rather than implying everyone entering treatment should expect it. The retatrutide evidence card (https://www.bodyhackguide.co/guides/retatrutide-phase-3-card) shows why different ways of counting participants can yield different results within a single trial.

## Eloralintide versus UBT251: don't rank these percentages

These are separate studies. Our comparison is deliberately about design, not a winner.

| Feature | Eloralintide phase 2 | UBT251 phase 2 |
| --- | --- | --- |
| Main receptor mechanism | Selective amylin agonism | GLP-1/GIP/glucagon agonism |
| Population | 263 US adults without type 2 diabetes | 205 Chinese adults without type 2 diabetes |
| Primary endpoint timing | 48 weeks | 24 weeks |
| Greatest reported mean loss | 20.1%, efficacy estimand | 19.7%, published phase 2 estimate |
| Placebo mean loss | 0.4% | 2.0% |
| Head-to-head comparison between them | No | No |

Sources: eloralintide paper (https://pubmed.ncbi.nlm.nih.gov/41207310/) and UBT251 paper (https://doi.org/10.1016/j.cmet.2026.09.021). Different duration, population and analysis make a simple percentage ranking misleading.

## Safety and development: what remains unresolved?

A placebo-controlled amylin study cannot establish “fewer side effects than GLP-1 drugs.” That would require a suitable direct comparison, including adverse-event severity, discontinuations and exposure time. We do not substitute a mechanism story for those data.

The phase 2 registry (https://clinicaltrials.gov/study/NCT06230523) records actual enrollment of 263 and study completion in August 2025. Lilly's 2025 announcement described future development plans; those plans are not proof of later trial completion or regulatory approval. This article reports the verified phase 2 evidence and does not present a company roadmap as a current approval record.

Durability after stopping, broader populations and longer-term safety remain separate evidence questions. For background on the compound, see our eloralintide reference page (https://www.bodyhackguide.co/compound/eloralintide).
